Stroke Recovery

My brother had a massive stroke six years ago leaving limited mobility on his left side. He has worked very hard rehabilitating himself with daily exercise on excercise equipment, physical and speech therapy, or any other means he can find to improve himself. Now after six years of a daily regiment of exercise he is experiencing an increase in muscle spasms in his left leg. The questions no one seems to be able to answer for me is, "is this a sign of deterioration or an increase in neurological activity?"
