HIPPA laws were enacted to protect patients from this behavior. The worker is in CLEAR violation of HIPPA laws.
Starting with the nursing home administrator would be the first step. However, it is difficult to find people to work in nursing homes (even the bad ones are hard to find) so you may just be given the standard "Thank you for bringing this to my attention. I will follow-up on this." I encourage you to monitor any further violations and report them to your state nursing home licensing organization if future violations occur.

It is illegal for a health care working to give out any information concerning a resident.  I would contact the nursing home director and bring your concern to her/his attention, before any further action.
