High TSH (TSH > 150 ) is not Affecting Weight (46 kg)

My wife has been detected Hypothyroidism before 4 months.
But she was not having Any Symptoms. Her weight was 46kg
At that time TSH level was 123.
We started medications. Althroxine 100 mcg.
Withine three months of time TSH level reduced to 0.68.
From last month she was taling 50 mcg dose
Today test result is showing TSH more than 150.
Still her weight is 46 kg.
So Can you tell me the reason for not heaving symptoms of Hypothyroidism & too much fluctuation of TSH Level?

Not sure why the dose was changed, TSH of 0.68 is fine, perhaps there was a concern that it was correcting too fast.  In any even, would resume the 100mcg dose and f/u in 2 months.  Some patients are surprisingly without symptoms -- but left without treatment, this is very dangerous and eventually all patients would have severe symptoms.

Hi Suketu,

Dr. Lupo generally doesn't respond more than once to any single question.

Congratulations that your wife is prenant.

As your wife is pregnant it is VERY important that her TSH is < 2.0.  WIth TSH so high there is serious risk to the baby and the pregancy, not to mention your wife;s health.

Your wife needs to immediately increase back to 100mcg and probably up to 125 mcg (pregnancy needs an increase by 20 - 40% in thryoid meds, as stated previously by Dr. Lupo).

She also needs to discuss her pregnancy with her endocrinologist as it will be important to monitor and adjust her thryoid meds.

This is very important!  Hope this helps.

Our doctor suggested two tests 1) TMA, 2) Free T4.
Result shows TMA is 300 and Free t4 is within normal range.
The problem is she has become pragnent. We are very much tensed.
