underweight lady with hypothyroid

Dear all
I have this query for my wife who is 27 years old  underweight (weghing 41kg or 90 pounds) taking decent diet everyday.

After her second pregnancy in Dec2007, her gynecologist had suggested to get the test done for thyroid. She is ailing with  all the major symptoms for Hypothyroid except weight gain. Her reports over last two years are as below

We consulted a doctor specialized in Int. Medicine and Endocrinology on 3rd Feb 2009 and his remarks as below

H/o child birth dec. 2007
S/c  hypo in 2008

Goitre OB          

He recommended following medicine
Thyronorm 50µg  half a tablet OD  for 7 days and then 1 tablet OD after 7 days onwards

But following the  above said medication for nearly 3 weeks , she started feeling to weak and sluggish. She was dizzy all the time . so we stopped the medicine and she was fine again.

Now with recent reports as per table above I am not sure how to move ahead in this regard. Any advice from your side will be highly appreciated

Would ask to be tested for adrenal insufficiency.
The thyroid can probably be observed without medication for the time being.
