Well I finally got my cytology report back today.
Two nodules were biopsed one 1.7cm and the other 2.7cm. I posted the US report awhile back. I had a multinodular goiter with two dominant nodules on the left. Both were mixed with internal vascularity.
Here is the result of the nodule that is 1.7cm. in the left lower lobe.
Most probably benign - consistent with a predominantly macrofollicular lesion.
Hypocellular specimen with follicular cells in sheets, microfollicles, macrofollicles and groups.
Rare follicular cells show cystic-lining cell changes.
Abundant macrophages are present.
Abundant colloid is present in small fragments.
Below is the result of the bigger 2.7 nodules in the left middle lobe.
Suboptimal cellularity but shows features suggestive of cystic degeneration in a follicular lesion.
Hypocellular specimen with follicular cells in microfollicles and groups.
Rare follicular cells show Hurthle cell changes.
Rare follicular cells show cystic-lining cell changes.
Abundant macrophages are present.
Scant colloid is present.
My test results are:
TSH 3.59 and Free T4 .86
TPO was 0.9
I am very concerned about the larger nodule result. Does "Suboptimal cellularity" mean that they did not get a good speciman? Also I am concerned about the "Rare follicular cells show Hurthle cell changes" Thank you and I would appreciate your opinion about how you would handle a patient with this result.
