I need help understanding an ultrasound report

I went to my Dr Monday morning with a very large painful mass on front left side of my neck.  I got the ultrasound report back in a form of a letter and I don't understand it.  It would so much easier if they would write this out for blonds. Here is the report:
Findings: Right thyroid lobe measures 4.9 x 1.5 x 2.0 cm and the left lobe measures 5.1 x 3.0 to 1.7 cm.  Right lobe is within normal limits without dominate lesion.  The left lobe shows a mostly cystic structure but with internal echoes and debris that measures 2.9 x 2.1 x 2.5 cm  Small lymph node is also noted in the left neck with does not appear to be abnormal.
Impression: Complex cystic structure in the let thyroid lobe
Comments: left thyroid cyst---see what the endocrinologist has to say.

Can someone tell me what does this mean?  Does this mean surgery?

I also just all my blood work back in today also. The ESR (SEDIMENTATION RATE) was 42, the T4 FREE is 1.19, T3 is 125, and the TSH is 1.63.

Would recommend seeing an endocrinologist for ultrasound guided FNA biopsy of this cystic mass.  95% of these are benign and most do not require surgery.  Likely there was recent bleeding into a small pre-existing nodule or cyst.  This usually improves in 3-4 weeks.
