Dear Dr. Lupo

I am a 37 year old woman, 5'0 115 lbs.  I was recently diagnosed with type 2 diabetes.  I have suffered from anxiety and migraines most of my adult life.  In recent years (past 5) I have been suffering from widespread muscular pain, chronic fatigue, shortness of breath, dizzines, ringing in my ears, eye pain, occasional numbness and tingling in my hands and feet, hot flashes, memory loss, and word recall difficulty.  I had my tsh checked several months and had a low tsh with normal t3 and t4.  It was re-checked and they were all normal.  I insisted on having it checked again due to recent extreme fatigue,so tired it hurts...and new levels are as follows:

rdw slightly low
vit d slightly elevated..it was low 6 mos. ago
last cholesteral 196
last a1c 5.8
iron slightly low
tsh .25
t3   2.96
t4 1.01

ultrasound results: mildly heterogenous in echotexture without sonographically detectable focal lesions. right lobe 4.5x1.9x1.7cm, left lobe 3.4x1.4x1.5c.,isthmus is normal in calibur 3mm, no evidence of adenopathy, no extrathyroidal mass identified.

Can you please explain?

The size of the thyroid is normal and there is no nodule.  The heterogeneity is nonspecific, but sometimes seen in autoimmune thyroid disease (ie Graves or Hashimoto's).  The TSH is only slightly low so at this point, would usually recheck in 6 weeks to see if a pattern is evolving, but there does not appear to be any specific thyroid treatment that is needed yet.
