metastic papillary carcinoma

my husband is uncertain about diagnosis surgery sounds pretty extreme he says if i dont need it why have it done ? my pathology report says no convincing malignant cells idetified, rare atypical cells of uncertain signifigance. 2#background huthle cells, colloid, and possible chronic inflammation.3#nci category-follicular lesion / atypia of undetermined signiicance. comment The right neck, mid thyroid nodule has some atypical cells of uncertain origin.These are not convincingly carcinoma, however.In contrast, the second speciman was obtained from a convincingly extra thyroidal lymphnode, and the finding of atypical papillary clusters of epithelial cells is very suspicios for metastic papillary carcinoma.These cells have rare nuclear inclusions and nuclear grooves are definatley observed.The architecture of fragment also has a suggestion of papillary growth.Therefor, the thyroid may contain papillary carcinoma that has not been well sampled.Further clinical and radiograghic correlation is suggested.JOYUS

Looks like the lymph node may be positive for thyroid cancer.  The key here would be to have slides read by an expert and have a careful neck ultrasound before surgery to map out abnormal nodes and plan surgery.
If the node is definite papillary cancer, then the surgery needs to be a total thryoidectomy, central compartment lymph node dissection and depending on where the node (that was biopsied) is located you may need a lateral neck dissection.

The FNA of the thyroid is most c/w Hashimoto's - so would test TPO antibodies as well and correlate with ultrasound findings.
