thyroiditis and new nodule

hello. i guess i'm still in recovery phase from the silent thyroiditis i have/had.  tsh went from .01 to .007 to 3.86 most recently.  i just had an ultrasound done.  as always, the thyroid is hetergeneous.  it's been that way for years.  however, now i have a nodule that is 7mm x 8mm x 4mm in the right lobe that appeared since the last ultrasound 6 months ago.  i've never had a nodule that i know of.  is this from the thyroiditis?  is it common for them to appear that quickly?

This is likely changes of thyroiditis - would repeat US in 6 months.

and why don't i have thyroid antibodies if my gland is so heterogeneous?  i'm antitpo negative.  my antithyroglobulin is only slightly elevated.  it's only a new points above the ref range..  a lot of the people on this forum have antibodies through the roof.

also, the new nodule is hypoechoic.  

i don't understand the difference between all of the "holes" in my thyroid, which is why they say the thyroid is hetergeneous, and nodules.  couldn't the new nodule just be a bigger hole?  or are all of the holes nodules?  i'm confused about the difference between a hypoechoic nodule v the hypoechoic holes.  they all look the same to me, but i'm just a commoner trying to understand.
