Hit by Hyper Storm?

So, I woke up, took my synthroid and by the time I got out of the bath I knew something was up....I always feel a little shaky and have a few faster heartbeats after I take synthroid, but by the time I got half way to work, I was feeling bad...It felt like my heart wanted to come out of my chest.  My face was going numb and so was my elbow...(why my elbow?)  So when I got to town, I took myself into the e.r. (beginning to wonder if I was having a heart attack)  I got lay down on a table and got hooked up and then left completely alone, for a long time...I'm guessing this is probably not a heart attack... Heart rate was 107 and bp was 159 over 95.  As I lay there, of course things started to come down and eventually I felt better, so I began to suspect it was being hyper....Er doc came in....he felt like I was on too much synthroid, but according to my numbers last week, I'm still a bit hypo.  He wants me to take 175 mcg of synthroid instead of 200 and gave me a script. He felt that going from 150 to 200 a few weeks ago was too big a jump. but he thought i should confer with my endo up in Des Moines.  So I contacted them and they said stay on the 200 and send the labs.  Labs will not be done until tomorrow...joy of life in a small town....I hope I'm not having a heart attack, I'll be waiting a long time.  
I have a home monitor...right now my heart rate is up a bit at 96  and my blood pressure is 139 over 88.  It was lower when I took my nap, which I'm about to do again.  I am wiped out.

MY GOD HOPE this is reading like a mystery book  i could not put it all down. i ran in to check what my dr ordered and she ordered t3 and free t4 and tsh. im hypo one month on synthroid 50mcg   and another draw in two more weeks.  i will pray for you that you get it all straightened out. GOD BLESS YOU and hang in there  please let us know what happens on monday.

I really wouldn't increase to 225 at this point after your experiences of last week.  Not until you see FT3 and FT4 and get to talk to your doctor.  I know that's easier said than done if he'll only write you a script for 225.  Maybe you can cut and re-cut pills to stay close to 175? 3/4 of a 225 is 168.75 - close to 175.  Just my opinion, but I know how frightening those heart issues can be.

Well it was getting to the point that all I have at this point is the 175 mcg that the emergency doc gave me.  Endo was suppose to send a script to wal mart for 225, but they only sent it for 25mcg...Walmart put a hold on it as it was a script change from 175 to 25mcg...they didn't want to allow it as they know that is too much of a drop...yeah walmart.  But now I have to call again on Monday and get it all straightened out....

If you really feel like you've exhausted all the local possibilities, then get on the internet and order FT3 and FT4.  There are some sites that will order the tests at your local lab and e-mail you the results.  

Which new meds are you planning to start on Monday?

My experience with cardiologists is that they are even more haughty than endos (no!).  If it doesn't go lub-dub, they're not interested.

HA No luck with my dr.  She was taught that testing T3 only complicates matters...She doesn't even like to do T4....TSH only baby......I just as well stay the course, see what happens when I take my new meds (have a prescription mess up now that I have to deal with on Monday)  Hope my heart doesn't blow up and move on.  Gal today will talk to a dr that was not there today about my heart issues...Good golly, now I'll be on a whole battery of tests for my heart and all they need to do is look at my synthroid...I wonder if a heart dr knows anything about all of this.

The thyroid doesn't convert T4 to T3.  It produces both T4 and T3.  Most of the conversion is done in the liver (I believe), some is done at the cellular level, and some other structures (like the pituitary) do their own conversions.  So, not having a thyroid does not impact conversion.

I agree that without FT3, it's much more of a guessing game.  Good luck with your call to your doctor.

But my thyroid is not producing...I don't have one...

There's a question... If you don't have a thyroid, what converts the T4 to T3?  I keep feeling as though I have no converter, so how can I convert....

I agree on the free T3 thing...it's just a matter of finding a dr that will do it.  Talking to my regular dr today about a new referral.

Goodness! You poor thing! That must have been very scary for you! I'd have to agree that the upper dose is more than likely the culprit. Personally I would stick to the 175 for a while. At least untill you hear back from your endo. A week even on the 175 will at least bring you back to a fairly normal rate. You are much like me with the sensitivity of evena small change in dosage.
  It really is important to get the Free T3 done though. It will pretty much be a hindsight thing. You having the symptoms will only show in the numbers that you are super hyper.
In the end it is up to how YOU feel. If you feel totally lousy on the 200 then 175 might be best. If you feel lousy on 175 then 150 might be too low. You may need to take a dose that is a tablet cut in half.
  TSH being 6.81 is still regarded as being hypo. BUT if your Free T4's are out of whack too, then you are probably more than likely on too high a dose. Confusing, for sure! The TSH really only shows that your pituitary is sending out the right amount of hormone to kick the thyroid into releasing the T4 tobe converted into T3. What is more than likely happening is that the thyroid is producing quite a nice dose of its own T4 and the extra you take is actually an overload. It may well be that you need a T3 supplement. If your Free T3 is low then you know you aren't converting the T4, but if it is high then you REALLY need to lower your medication!

When she moved me from 150 to 200 my TSH went from 20 to 6.81...where will it go with another 25 mcgs.....Can you go negative on these scales....

The increase from 150mcg to 200mcg is TOO high, TOO fast.
I would stay on the 175mcg to let the dust settle...so to speak.
Your body needs time to adjust to the increase and going that hard will cause heart palps.
Not a nice feeling either.
Demand the Free T3 as it IS important that it be done.
My guess is the 200mcg dosage has bumped the FT3 right up...hence the heart palsp.
Let us know how yr labs come back .

You can't be left with too many thoughts and swing those meds around... You need to follow a certain dosage correctly.

As for the testing - you are getting just a guessing game when you T3 is not evaluated - so the doctors you ahve right now are NOT looking at you with your best interests and are messing with you to see if you regulate blindly.

You need T3 tests- especially if you are palping to the degree of sending you to the ER. Your heart is very strong but too much abuse and not correct thyroid tests - will take that muscle down.

From TSH being 20 to 6.8, I guess there are some differences.  I still get cold, but sometimes I get hot too.  I still get sleepy, but maybe not as often.  I don't fall asleep as much when I am watching the news.

Didn't know that about the T3.  I have yet another issue.  The ER dr told me to drop to 175 and gave me a script.  The big city dr said don't change and wait til they get my records from my hospital.  I know it was faxed this morning, but they never called me.  I'm afraid to take the 200 again, so this morning I went for the 175.  My face still went numb, but I didn't notice the racing heart.  I guess I am on my own.  I have talked to 8 drs about my thyroid in the past 7 months.  You'd think I could count on one of them.

I have to agree that it seems the meds increase was just too much too fast.  I know how frightening that can be...when I was started on too high a dose, I started having heart symptoms, too.  

Since the increase, have your hypo symptoms improved at all?  Seven weeks is enough time for the level to have stabilized, but your FT4 is still quite low at 1.03, and, of course, TSH (for what it's worth) is high.  I'd really demand FT3...especially after the heart issues (T3 is the biggest factor in heart regulation).

TSH was 20
Free T4 was .94 I'm thinking.
and they refuse to do Free T3.

Last week
TSH was 6.81   Normal being .5 - 5.0 (still old school here)
Free T4 was 1.03  Norm .71 - 1.85

I have seen several dr's and have yet to find one to do FT3, but I have now found two that will do the FT4...lucky me.  

I'm on week 7 since meds went from 150 to 200 mcg.

sorry to hear about the hyper stuff.  what was your tsh, ft4, ft3 when before your doc increased your dose to 200?
