How concerned should I be about the following Thyroid US Results?

The following are my Thyroid US Results. Can you tell me what may have cause the mildly diffuse enlargement? Also what is the chance of the cause being cancerous with these result findings? Size right thyroid lobe: 4.7 cm Size left thyroid lobe: 4.7 cm Size isthmus: 0.3 cm
Texture: Heterogeneous
Estimated total number of nodules greater than or equal to 1 cm: None
Number of spongiform nodules >/= 2 cm not described below (TR1): 0
Number of mixed cystic and solid nodules >/= 1.5 cm not described below (TR2): 0
There is no significant focal thyroid lesion identified. No dominant mass is evident and there are no microcalcifications seen. No additional contributory abnormality.
IMPRESSION: Mild diffuse thyroid enlargement may represent mild diffuse goiter. No indicators of malignancy are identified on this exam.

The thyroid enlargement could be caused by Hashimoto's, Graves Disease, hypothyroidism or hyperthyroidism.

There are no significant lesions or dominant masses and the report says there are no indicators of malignancy, therefore the chances of cancer would be, basically, zero... In other words, there is nothing significant about your report and nothing to worry about.

That said, what prompted you to have the ultrasound?  Are you having symptoms of a thyroid problem?  If so, what symptoms and what problem do you think you might have?

What, exactly, is your normal TSH level?  Is that the only thyroid-related blood test you've had?  There are other tests you should have that are more important than TSH... those include Free T4 and Free T3 as well as antibody tests appropriate to whatever symptoms, if any, you might have.

I have been having heart palpitations, hot flashes and sweats for about 8 years now that have been medically dismissed as menopause symptoms.  These symptoms have increased and have cause me to become fatigue, and unable to stay sleep at night. I also have a changing taste on my tongue that sometimes has alternating cinnamon, salty, and bitter taste. In Feb 2018 I underwent skull based surgery to remove a Glomus Jugulare Tympanicum tumor that my neurosurgeon expected to help with the Thermal Sweats and Palpitations as he thought the tumor was causing them. However, there was no change except the taste bud symptoms began. My PCP did a physical exam of my Thyroid and feel it was slightly enlarged. She also ordered the following test, which I have included my results.

Correction I see fluctuations with and increase in the same month, as well as a decline from the highest level.

This would have been post surgery. My F/U MRI of my hypothalamus and pituitary region results were  as follows:                                                                                                
Skull Base: Hypothalamic and pituitary region are grossly normal.
Craniocervical junction is normal. Previously noted area of marrow
replacement involving the dorsal right mandibular body is not well
visualized on this current exam and may have been related to periodontal
inflammatory change. No definite findings of marrow replacement along
the right mandibular ramus or abnormal asymmetric enhancement along the
course of the right mandibular nerve in the visualized mandible to
suggest perineural spread. No evidence of other significant marrow
replacement process in the skull base.
Postoperative changes the right mastoidectomy and resection of the right
temporal bones and jugular foramen mass. Mildly enhancing soft tissue in
the mastoidectomy defect adjacent to the right jugular bulb, most likely
representing postoperative granulation tissue. No definite findings of
residual or recurrent paraganglioma.

There are a lot of things that affect TSH and cause it to fluctuate and the fact that you had skull based surgery could certainly have been an influence on it.

Your Free T4 indicates that your thyroid may be beginning to malfunction as it's somewhat low...

Have you had a thyroid ultrasound to check for nodules/swelling on your thyroid?  

Also, cortisol testing would be advisable as high cortisol levels can cause the symptoms you report.
