Thyroid

Thyroid gland is diffusely hypoechoic and heterogeneous with hyperechoic bands. There is evidence of increased vascularity. the right lobe measured 5.5x2.7x2.2 cm
the left lobe measured 5.4x2.8x2.1 cm
thyroid isthmus is 0.5 cm in thickness. what does this mean?

grave can go to reduction of thyroid gland but hashimoto dosen't

I do not have % of patients who reported reduction of the goiter when on medication; in most cases the hormone replacement therapy was delayed.

There is no officialy approved treatment for Hashimoto's thyroiditis other than hormone replacement therapy.
The swelling may get down when your TSH will be brought to normal (below 3); because most doctors are treating by the numbers you most likely will be given T4 pill.
For now you can monitor the circumference of your neck in by measuring every week now and when on medication.
You can also take the picture of your neck every week with head tileted back to monitor swelling size and condition.

So how is this treated? will my neck eventually go down or will I have to learn to deal with it?

Most likely this looks like Hashimoto's thyroidis

I did have blood work done. My Peroxidase (TPO) AB was 193, My T4 Free was 1.04, and my THS was 13.860. I see people posting about T3 which I'm not sure what that test shows but, I don't believe they did that one.

Diffusely hypoechoic and heterogeneous with hyperechoic bands This indicates of inflalmmatory process or overstimulation perhaps by TSI antibodies. Increased blood flow in the whole gland may be an early sign of Graves disease or intense thyroiditis
The thyroid volume per Brunn, et all
left lobe approx 15 cu cm (ml)
right lobe is 14 cu cm
total is 29  which is almost 4 times the average size (goiter)
thyroid isthmus is 0.5 cm in thickness 2 times bigger than normal.
You may need a blood test to find what's going on
