Bladder pain or something else?

I am a 34 yo female. Until about 3 weeks ago I had a Paraguard copper IUD that was placed in 3/07. A previous IUD placed in 12/06 had expelled in 3/07. The second IUD was going fairly well until in mid August I visited the gynecologist with a combination yeast, bv, uti. They were not all diagnosed at once, but over the course of a few weeks. I had engaged in protected sex a week prior to the incident with a clean partner, but I believe the oral sex got a little “messy” and perhaps set some of these things in motion. I also want to mention I am an avid mountain biker and bike 10-40 miles per day.  I never had any issues like this before. I have been riding my bike since I was a kid, etc. I should also mention that all the doctors ask me about back pain – in 2003, I was in a horrible car wreck and fractured and messed up my back. So my back pain is 24/7.  Also tested negative for diabetes.
So we cleaned up all my vaginal issues and removed my IUD. I had to push for this because I was having yeast/bv cycles and I was very uncomfortable. Finally the gynecologist told me that everything was fine. Based on negative urine cultures and vaginal cultures. All STD tests and a pap came back fine. I was tested for HSV1&2 twice and they were negative. My outstanding issue is some pain, burning, tingling, clitoral burning/pain/itching and UTI type symptoms. I went to see a Urologist and he did a the camera deal and looked at my bladder. He saw no visible signs of  IC but wanted to try me on Urelle 4x a day and see where things lead. For about 3 weeks the Urelle has been a major help. Urinating is definitely more comfortable, and I have been minding a decent diet with no soda/coffee/alcohol, etc. However, to some extent I am still experiencing the above symptoms. As well as a stabbing pain right above my pelvic bone on my left hand side. I also sometimes feel that my vagina/labia/urethra are sore, itchy, dry, very bothered. I do a cortisone cream and it helps a tiny bit. I do notice that the area seems a little more red than usual but no sores, cuts, abrasions, etc. The only doctor who is still willing to see me is the urologist, he thinks he can fine tune things with or without Urelle, but my pain and symptoms he is not sure about. Also since January I have had a flare of internal and external hemorrhoids as well and will be going in January to have these addressed. I believe it is from all the biking, although the Urelle seems to have caused some constipation.

My questions are: who should I be seeing for these issues? Do you think they are at all related to IUD infections or inflammation? PID? I read about pelvic floor disorders and vulvodynia and clitodynia, etc….Should I see someone to rule this out? What about skin scrapings of the labia, etc. or colonoscopy? Etc? What is the best course of action to take to get the best plan of action to correct the symptoms or just a final answer that could be live with the symptoms?  I believe STDs have been ruled out, but  still feel there is some infection/inflammation/bacteria that is causing me issues and worry I could pass it to someone else. I have great health insurance so getting tested is not an issue. Just need some direction of what type of doctor to see and what tests or suggestions to run with. Thanks.

i got my copper iud in 07/01 and just got it taken out 09/24/09.  i had the same problems u did and i leaked urine. i would take anitbiotics and that would help for a while, but it would always come back. nothing ever showed up on the cultures except yeast and i would blame it on my husband and it almost ruined our marriage. i truly believe it was from the iud. my symtoms r gone without having to take antibiotics this time and i dont have the back, pelvic, or just the feeling that something just isnt right anymore since getting it taken out. its just not for some ppl. i should have gotten it taken out earlier, but i just put 2 and 2  together and got the right answer.  they need to start making something for men other than condoms or the snip. either way they cry about that. we have the periods, the children and the hassle of birth control. somethings gotta change.  but have it taken out and c if that helps before it does some major damage. i did and i wont ever get another one. im 40 btw with 2 children and i will just have to be careful and try not to get pregnant again. if so i think its better than the mess i was going through with iud.

....back pain lead to a sonogram, cat, and subsequent mri where it was suspected that indeed the kidny was in play, however upon further examination it was identified as a L1 spinal schwannoma. still having bladder pain and dysuria. any idea if this nerve and schwannoma is behind the dyuria and painful voiding?

Believe it or not, you may want the urologist to check for kidney stones, if he hasn't already. My sister (22 years old), cousins (26 & 33), and aunt (52), have all had issues with kidney stones, and the symptoms you described were very much what they went through. So, if he's still willing to see you, I'd keep pushing with the urologist. Good luck.
