Middle back pain on right side

I have middle back pain on right side which extends to right shoulder and right hand and even  sometimes in sides of the chest.
i have consistent cough even though in a small amount but for a long time.
what could be the possible cause.i have not yet gone to a doctor a s i thought it might be due to wrong sleeping habits.
please tell me the possible causes and what treatment i should follow

thanks for quick response i am a regular gas patient and have taken medicines lot of times but no affect.
i am a smoker from past 4 years(3-4 ciggrates/day) .is the risk related to smoking.

Pain in the middle back and under right shoulder blade is usually due to gall stones, duodenal ulcer and liver infection. Hence an ultrasound, liver function test and an upper GI endoscopy is needed for diagnosis. Cough could be due to gastric reflux. Raise head end of bed. Take Omeprazole empty stomach in morning and an antacid gel at bedtime. Refrain from smoking, alcohol, fuzzy drinks and spicy food. Walking around also helps clear gas. I would strongly recommend you to see a gastroenterologist (a specialist who looks after the diseases of our digestive system) if these tips help you. Other than that it could also be costochondritis, pleuritis (inflammation of covering of lung—needs to be checked as you have cough) or pericarditis (inflammation of covering of heart). Since you do not mention any other symptoms, it is less likely to be any of these.
Please consult your PCP for primary examination followed by proper referral. Take care!

hello there back pain with cough can indicate infection usually. honestly thow a trip to your doctor would be best because an infection will only get worse.
