Pain from Pilonidal sinus

Hello,

I have a question about my Pilonidal sinus. Lately I have been having pain in my Pilonidal sinus which is just above my gluteus maximus. For the last two to three weeks I have been experiencing pain. My GP put me on antibiotics for 10 days now and no change. He said the area that is visible is not swollen from the outside. There is no puss or discharge. I've had the Pilonidal sinus for as long as I can remember (I'm 30) but this is the first time I have had pain.

The pain pretty much happens when I sit. When I stand/walk/run/lay-down there is no pain. But when I sit in a chair or drive, it hurts.

Is there anything else I can do to clear the infection if the antibiotics do not help?

If there is pain when you sit, then your tail bone or coccyx is involved in some way. First go for X-ray and then for MRI. The sinus tract may have deepened too and this could also be causing pain. Infection anywhere in the sinus can also cause pain. It could also be due to bone disease or, nerve compression. A combined approach of a neurologist and an orthopedic specialist is needed.
Please consult your PCP for primary examination followed by proper referral. Take care!

Sorry, just wanted to clarify about the black in the Pilonidal sinus. Its only the size of a pin-head, not the whole area is black. Only where the hole actually is (its been like that as long as I can remember into my childhood).

I can confirm the pain is mostly where the tailbone sits. The Pilonidal sinus looks a little black where the hole is (but its been like that as long as I can remember into my childhood).

My doctor has refereed me to a General Surgeon/Hepatobiliary Surgeon (specialist). He seams to think its because the sinus tract is deepened. Is the correct specialist I should be seeing (General Surgeon/Hepatobiliary Surgeon)? I have not had any scans/MRI's done.

I must admit, today the pain is not as bad, even though I am sitting most of the time. Also, on the weekend I was lying on the couch or watching TV in bed most of the time and there was no pain for half a day when I went to work again (I sit in a office).

Interesting idea (sitz bath) yet I am not sure what it will achieve because the doctor indicates that the external does not seam infected.

I'm only guessing here, but have you ever considered a sitz bath?  Those are also used for hemorrhoids.  Epsom salts or baking soda in a sitz bath might be helpful.
