Please tell me what it is and how to fix it? Please be detailed.

Im 23 male and i smoke half a pack of cigarettes a day. No drugs no sex no nothing. These symptoms have acumulated since i was 16. But now lifes just unbarable. I sweat at a persistent rate (not like drenched but a simi thick oily sweat) it makes me smell awful even if I'm in cool environments (2-6 times a week). Maybe twice a week i have a tingling sensation thru out my entire body especially in my chest, but most of the time its in my hands (4-5 times a week). U have a fast heart rate. heart palptations. Insomnia (writing this i havent slept in 3 days,in a week i may sleep 3 days often only a few hours). Extream isolation. I sometimes use a razor blade and cut myself ( monthly). A lot of time feeling really sad and feeling like I'm worthless and will never be anything. Cant sit still (when trying to sleep my feet flail and my arms do it sometimes). I cant remember simple things even if important. My thoughts like run together and cant form a simple thought line. Offteb forgetting what im doing or thinking, slow thoughts. Unable to process information. Always see the worst possible in things. I see bugs crawling around my room. Hearing voices telling me to hurt myself, how I'm worthless, how i should kill myself, to do things. ect.. when i drive i feel like im being followed, or somethings going to kill me if it catches me. Seeing figures watching me, evil demonic morphing shadows taunting me on walls (mostly in the dark). Iv push all of my friends away, no relationship with family, I block any attempt anyone makes. My day to day life consist of watching tv and laying in bed all day 24/7, no human contact. Believing things like i once said i used to be a bat so i can screech really loud or that i was born in the 1800s and fathered adolf hitler. Conpulsive behaviors like whipping my face. I talk to myself and never remember what i talk about. Large fear of social interactions and scarred to go in stores (feel like everyone stares at me). Excessive masturbation(4-8 times a day sometimes more). I often feel like i got something in my chest and cant cough it up. Feeling like somethings crawling all over me. Suicidal thoughts (like twice monthly) iv also attemptted suicde 2 times. Fatigue. Self dbout. Alwasy tierd. Fast heart pace. Extreamly low self-esteem. I also sometimes cant concentrate or focus. Times of intense happiness or sadness. Cough from feeling like somethings in my chest. These are all the symptoms I have written down over the years. My question is what could all this mean? Am I slowly dieing? This has made my life absolute hell. If something don't happen really soon. I'll activate my escape plan. Should I seek help at emergency room?this is no quality of life and I wouldn't wish it on anyone ever. Please be detailed in your answer to these problems, in what it could be caused by ect. Please help me! much appreciated and thank you.

This sounds like severe depression. I do have suicidal thoughts, so I pretty much have an idea of what your thoughts might be. You should go to the emergency room.

Nicotine is a drug, for your information. Also, it doesn't seem like those cigarettes are doing anything to help this. You should also probably get your chest checked because cigarettes can cause cancer.
