Should I be concerned about small amount of bence jones protein in my urine?

I was sent to an immunology doctor a few months ago due to having frequent sinus and chest infections going on for more than 10 years. I am 27 now. Her initial blood work only showed my IgM was elevated. IgG, IgE, IgA was all normal. She ordered further testing. I was viewing the results online and it said a small amount of bence jones protein was detected in my urine. It also states the clinical significance at this level is unknown and suggests repeating in 6 months. Also from the urine it says An ill defined band in the gamma globulin region is detected. My serum results did not show anything with the immunofixation or electrophoresis. It says Serum protein electrophoresis does not suggest a monoclonal pattern. My serum light chain ratio is normal, free kappa and lambda appears to be in the normal range. Calcium levels are normal. Creatine levels are normal. Total protein in the blood seems to be right in the middle of the range. The Albumin, Alpha 1,2 and Beta 1,2 Globulins are all at normal levels. IgM is still elevated (2.43 g/L) but is lower than last time. Also, gamma globulin blood levels are normal range, but in the urine there was an “ill defined band”. I do have fatigue, I do also have tenderness in my chest and back from time to time but I always kind of assumed this was from exercise. With all I have said how concerned should I be about the bence jones proteins in the urine? It did not list an amount by weight, just said small amount was detected. Also, since I forgot complete blood count everything is within normal levels. I plan to call the Doctor Monday morning, as my follow up is not until October and with my generalized anxiety disorder this has really been affecting me. A while back an osteoma was detected on a sinus CT, but was thought to be nothing. Would that have anything to do with this?

Well, it' my understanding that you shouldn't have bence jones protein in your urine.  That it is a scant amount and you are watch and see is good.  That's a good sign that all will be well.  Repeat that test in 6 months as they've asked.  If the amount increases, it could be indicative of a problem.  There are concerns about multiple myeloma.  This can go undetected as it has few or no symptoms for a while but does shoe this type of protein in urine.  https://www.healthline.com/health/bence-jones-protein-quantitative#use  Now, your test was not actually positive.  It is a watch and see situation.  So, I would not worry at this time. There are things that cause this besides the more serious things.  So, watch it and be aware. Don't miss a doctor's appointment. The band (ill defined) is related to the protein they found.  That's part of that.  Just like they found a tiny amount of protein, they found ill defined and inconclusive bands.  All levels are low.  I am sure your anxiety is not helped by this finding.  But they are NOT moving forward currently which is a good sign that they are not concerned.  So, tell us what your doctor said and we'll talk further.

I saw my family doctor yesterday and he is having me do a 24 hour urine collection test. Later in the day the specialist who ordered the tests also called and will be seeing me next Friday at 4. Hopefully the 24 urine test results will be ready in time for the appointment. Overall my family doctor did not seem too concerned by it. He said if it shows the protein again I would likely be going for bone x rays next.

So, the 24 h urine test also ended up showing a "small amount" of bence jones protein. Tomorrow is my appointment with the specialist so it  will be really interesting to hear what she has to say.  It still doesn't say exactly how much I have, just that it is a small amount. I really hope it all ends up being nothing.
Just in case anyone was wondering the diagnosis I got today was Lupus and MGUS.
