UNRESOLVED PROBLEM

I'm a runner and cyclist who normally runs 100-150 miles and rides 150'sh miles per month.  At least, I was up until recently.  I am currently 46 yrs old, 5 ft 9 in, weigh 170 lbs, and up to mid-2013 was in the best shape of my life.  Over the course of the last two years, I've watched my fitness level and physical/mental health  the following symptoms have began to eat away at my physical and mental health:
- My fitness level has degraded and degraded to the point to where I struggle to run a 5K (3 miles) where I use to be able to easily run 10 miles (in early 2013) with very little effort.
- No libido.
- Tired all the time.
- Only sleep 2-3 hours at a time.
- Hungry all the time.
- Constant brain fog.
- Sometimes I forget what the hell I'm saying or forget a simple word right in the middle of a sentence or train of thought.
- My memory has gotten worse.
- My vision has gotten worse.
Over the past year to year and a half, I've been to dozens of doctor appointments, have given gallons of blood for testing, have seen an Endo a half dozen times and still am not anywhere with this.  
As far as blood work:
- T count runs from 350-400 but Free and Total T have consistently test low.  
- Free and Total T3 and T4 numbers are usually low.
- TSH is normal.  Which, I don't understand ... if my T3 and T4 are low, shouldn't my TSH be high in an attempt to raise my T3/T4?
- Pituitary and Thyroid scans are normal ... At least I'm told they are.  Probably read by a radiologist and not a doctor.
- Some of my early Cortisol tests were a little high but seemed to not have been a concern for the Endo I was seeing.
- My LH and FSH levels tested low in the beginning of my journey but haven't been tested since.
I was on Fortesta T-Cream for about 6 months and it did absolutely nothing for my libido or any of my other complaints above.  I have also been and am still on Synthroid and Cytomel ... which haven't solved anything either.
So ... what do you think could be the culprit??? ... Adrenal Fatigue ... Pit Adenoma ... Thyroid issues ... or am I just old and trying to fight a losing battle to Father Time?

I've consistently been right around 1.0 on a .4 to 4.2 ref range.

What was the TSH result you were told its normal? Do you have the numbers?

If so, what would be your recommendation for HRT? ... 200mg of Cypionate every 2 weeks, 100mg every week, 200mg once per month? ... ???

Hi John. If you were a patient of mine I would give you a trial of Testosterone Replacement Therapy regardless of what your T levels are because these can never be relied upon to diagnose Testosterone  Deficiency Syndrome.
If you went on a trial of something like Testogel (depending on where you live) you would know in a matter of a few days whether you were on the right track or not. But don't expect miracles and don't push yourself too much either.
