Chronic diarrhea can be due to parasitic infestations. Other causes are irritable bowel syndrome, celiac disease, crohn’s disease, malabsorption syndromes, pancreatic diseases, diabetes, hyperthyroidism etc. HIV too is a causative factor.
I think you should discuss these possibilities with your doctor. It is difficult to comment beyond this without examining. A comprehensive investigation is required keeping all the points in mind.

is it only after you eat and is it almost immediately after you eat? If so, it could be your gallbladder......I had to have mine taken out
I would ask your doctor if Niacin (1500 mg per day) might help you. You might be missing something that this might help. However, it does cause flushing, so a lower mg to start might be advisable, and also eat a larger dose of fibre (apples, potato -with skin, lettuce, broccoli, etc.) as it tends to constipate you a bit, but if you can find the fine balance, it might help you.
