what is this pain?

i have a pain under my left breast that goes around to my shoulder blade and if i have to cough the pain is rea sharp beside my shoulder blade. what is this caused from?

Hello,
Two possibilities are important in your case. These include muscle spasm and muscle strain and secondly any pancreatic cause needs to be ruled out. I suggest you to consult a physician and get an ultrasound abdomen, CT KUB (kidneys, urinary bladder) and serum lipase and serum amylase done. Till that time, you can apply warm compresses on the area and take some anti inflammatory pain killer after consulting your doctor.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. I agree with mjeff about the concern for cardiac issues any time there is chest pain. There are many factors associated with your symptoms. Since the pain is worsened by cough, it could be an intercostal muscle strain. Also, pleurisy could present this way. Since the pain is as high as the scapula, I'm not sure about "gas pains'. A hiatal hernia, maybe.

I'd say you might have gas.  Sounds funny, but gas can travel around and cause extreme discomfort.  If it does not go away I'd within a few days I would seek medical attention.  You would not want to let this go too long because it could be something more serious i.e. heart attack.  Are you aware of your cardiac health and risk factors?
