help me understand

For the last few weeks i have been having alot of pain in my back..mostly on the right side and the pain will go under my rib cage...i dont have a kidney infection, no pain when i go to the restroom, no fever, and there is not pressure...i have had stones before but have not passed any in about 2 years....went for an ultrasound and was told i have a swollen ureter and it is infected...how is it that a urine test did not pick it up and what is causing it?

Thanks so much..they ruled out stones but did not go into the bladder just the kidneys...Is an ultrasound of the Bladder needed or another test?

Your post suggest you have a dilated ureter. This can be caused by a number of things, and should be evaluated by a urologist.  Stones in the ureter, bladder outlet obstruction with ureteral reflux, a mass in the pelvis causing obstruction of the ureter, etc.  If infection was causing the dilation one would expect a urinalysis with culture to be positive.  In absence of infection other causes need to be ruled out.
