Bladder or Kidney problems?

In March 2004, I woke up one night with excruciating pains in my left flank area as well as frequent urination – 40-60 times daily 8-10 times nightly; extreme fatigue “flu – like feeling”; shortness of breath; chest pain; palpitations,  abdominal discomfort/pain; nausea, vomiting, diarrhea, constipation intermittently; numbness on feet; abdominal pain/bloating along with muscle pain & tenderness.

They said I had a UTI and treated it with Levaquin, but I kept having these problems and all of my testing says nothing is wrong - also, my blood tests are completely normal.  I don't feel like I have UTI's but feel like something wrong in my kidneys with frequent flank pain, mainly L side but R side sometimes also.

I've had Abdomen - Ultrasound, Transabdominal and Transvaginal Pelvic Ultrasound, Ultrasound - Right upper Quadrant, IVP w/Nephrotomogram.

The results said I had a linear area of lack of contract enhancement involving the upper pole infundibulum of the right kidney which is not persistent – possible crossing vessel; Enlarged Liver; diffusely echogenic; pancreas was not and distal aorta visualized due to bowel gas.

In 1997 I developed Group Beta Streptococcus; which turned septic and wonder if this is related?  What could be wrong with me, any ideas?  Bladder, kidney ...uti problems or something else?
