Difficult penetration led to penile abnormality/symptoms. What happened & what now?

Years ago during 1st-time sex, I had difficulty penetrating followed by discomfort and trouble maintaining an erection in the days after. Shortly after, I developed frequent urination with a partial spraying stream and often feels that I have not fully emptied. I noticed that the penis never fully retracts anymore (and very slightly curved up) and putting any pressure on the penis head downward (toward the outer ends of the corpora cavernosa) is very uncomfortable as it remains less hard there during erections. It is as if the penis is made of 2 parts now, with the outer one kind of limp. Also, my urine lacks bubbles since (it used to have lots). The glans at times feels somewhat numb and stroking the penis is not pleasurable (though orgasm is). Cystoscopy, MRI, physical exams have shown nothing - though I can see a physical difference. Is there any hope of a diagnosis or treatment?
