Rising creatinine levels can be indicative of worsening kidney function - especially in the setting of diabetes. Obtaining urine tests can be done to look at the 24-hr urine creatinine and protein. Imaging the kidneys could also be done to exclude anatomic abnormalities.
These options can be discussed with your personal physician. I would also consider a consultation with a nephrologist (kidney specialist).
