Left testicle pain during masturbation/sex. Also groin,flank,back pain, plus gas.

Male, 37 years old in otherwise good health. And sorry for the long post.

This started about 6 months ago and I see no end to it. I noticed at the time that during masturbation or sex, I had a weird feeling in my left testicle followed by dull pain. I checked and saw that while masturbation/sex, it was riding way up into my groin area. The dull pain and uncomfortable feeling would go away soon after I ejaculated, and the testicle would descend back into the scrotum, so I did not bother much.

Then one day, while masturbating, I felt a jolt of sharp pain in my left testicle area and it was soon followed by pain in the groin, which then spread to my left flank and left abdominal area, down my left thigh, and my back was also soon cramping all over. I went to see the GP the next day, and he checked and told me to get an ultrasound of abdomen. Said it was just gas that got trapped and gave me some meds for the same. The tests also came negative for anything wrong in that area.

I told him about the behavior of the left testicle and he said wait and see if it happens again. Sometimes it gets pulled during sex/masturbation but usually heals itself. I did the same but after about 2 months (a little earlier), this happened again. This time my left flank looked noticeably bigger than my right flank, like I had a love handle on the left side only. I also got a boil in the area between the left scrotum and the left thigh. So, GP gave me antibiotics and asked me to see a Urologist. The urologist also did some tests for urine, blood, etc. and ordered Ultrosound of testes and abdomen, which all came back negative for anything wrong. Urologist said the infection was just in the hair follicles and not related to the other symptoms. Happens sometimes to people who shave their pubes. He gave me some antibiotics and sent me on my way, saying I was otherwise fine.

But I am not fine. This cycle of sharp pain in testicle > groin pain > flank pain > ab filling up with gas > back pain > thigh pain > boil , is getting quicker and quicker. Now it happens every 3 weeks. I went to the Urologist a couple more times but he maintains there is nothing wrong, and finally asked me to see an Orthopedic Doctor saying it might be a muscle issue (probably wanted to be rid of me).

I am intensely worried about this. I have always been a sexually active person and never had my testicle ride into my groin area, and now this issue makes masturbation or sex almost impossible without worry. I pee fine, and I ejaculate fine, and there is no swelling in the testicular region, but my left testicle rides way into my groin area during masturbation/sex and I constantly worry I will set off another round of above symptoms.

I am asking here as a second opinion / fresh set of eyes on the issue. I am worried I have something worse than what the doctors are saying, but its not getting diagnosed. I would appreciate any help with this.
