Nothing seems to fit my symptoms

I am a 52 year old circumcised heterosexual male.

For several years (3-5) sex has been painful. The frenulum breaks and the entire underside of the head (corona) becomes like a membrane - bluish white, and looks like a heated sausage skin that is stretched to the point of breaking - This area is THIN and easily ruptured. The stretching of erection tends to make the skin break. Bleeding can occur and there may be more than one areas of rupture/breakage. Breakage occurs only as a result of sex. Thinning of the corona is constant.

November 2008 the odor of my urine changed, strong, non-ammonia, sweetish. There is no other symptom. No cloudy or burning discharge. My scrotum area has a tingly itch that is constant, although that is more recent than November. I drink a tremendous amount of Orange Juice (concentrate). When the odor occurred I changed my drink of choice to Cranberry (sweet concentrate) and the odor actually disappeared for a few days but came back. Before adding cranberry it was a constant odor; since it is about 50% of urination episodes.

I am using hydrogen peroxide to treat/ cleanse the entire area daily. I am taking no medication other than Excedrin for occasional headaches. I have used Vitamin E and Tea Tree Oil creams with limited results. Occasionally I will use Vaseline Dry Skin Lotion. My bath cleanser is Dove - bar and soft.

I have had rare episodes of HSV treated with Acyclovar and penile yeast. I have to monitor my sugar as it tends to run 95-120.

I've tried to be as inclusive as possible so a positive identification can happen.

Thinning of penile skin can occur with a skin change called ballanitis xerotica obliterans. This may respond to topical steroids. The recurrent frenular disruption sounds particularly painful. It may be reasonable to surgically divide your frenulum transversely and suture it back together in a side to side fashion. This will take the tension off the frenulum and it will be less likely to rip.
Using hydrogen peroxide will certainly be drying and damaging to the skin. I think that you should cease using it. Just daily gentle washing with soap and water should be fine. You can use a non-scented hand cream to help remoisturize the area.
