Post Orchiopexy problem

I'm a 22 year old male who went recently went under a voluntary orchiopexy procedure almost 7 weeks ago to correct a problem i had where my testies would rotate at night.  I now think that my urologist never properly untwised my left testicles spermatic cord all the way out before he stiched them in place. Could you explain to me the correct way the doctor is rsupposed to do this procedure.I was under the impression that the doctor was supposed to take the testies out of the scrotum during the procedure and make sure they were not twisted then stitch them in place. After the surgery I was only left with a half inch incision in the bottom middle of my scrotum. My left testie now permently It hangs horizontally when i stand and i generally feel nauseus since the surgery and have been having pains in my abdomen and along my inguinal canal.I also have this feeling that i need to rotate my left testie 360 to the left when i stand. My right testie hangs vertically and generlly have do not have any problems with it.  I have had 2 follow up appointments since the operation and have told my doctor my sympotoms and  he confirmed that it was hanging horizontally but he just told me i was fine and my symptoms were just do to inflamation and told me to take ibuprophen. I have also had a ultrasound since the surgery and urine test which all came back not finding anthingthing wrong. However i have read that ultrasounds are not 100 percent accurate when it comes to partical torsion.  Any advice on whats going on here or what i should do about this would help.

My pain in the abdomen is more specifically my lower abdominals and oblique muscles. There is a very weak feeling and sick feeling that started out as a burning in my spermatic cord and obliques post surgery which later became a numb weak feeling. It becomes more apparent and bothersome when i exercise where i find it diffficult to contract and expand my lower abdomen causing me to breath shallow. Mind you i didn't have this problem before the surgery. And like i mentioned before I get relief when i try to rotate them over. I get this strong fullness felling back in my belly and i'm able to breath easly again and my testies feel better over all. The only other thing that might be telling is that the front of my testies faces inward toward the other testies instead of facing forward like the medical pictures show.

Dear Roadtrip
Ultrasound can not tell you about position of the testis and doppler is only helpful in complete torsion. In your case posiibility of partial torsion can not be ruled out (unfortunately none of the investigation has high accuracy here). AS far as pain in abdomen is concerned (I presume nothing was done in abdomen/ inguinal canal in your case) it could be due to stretching of the spermatic cord, which I think will become normal with time. However for testis pain possiblity of 2nd look surgery may be required.
If you have burning during urination/ pain in flanks of abdomen/ blood in urine please rule out the possibilty of stone in urinary tract which can cause pain in testis (and Nausea).
Best wishes

If the ultrasound shows normal blood flow, I doubt that the issue is one of a partial torsion. With a partial torsion I would expect that your testis would have died by now. The fact that your testicles have differing lies and positions within their respective scrota most likely  reflects their differing anatomies.
But what is the cause of your pain/nausea? By now I would think that you would not be having pain or nausea. Your sensation of needing to untwist you testis is unique. I have not experienced a situation in which a patient has told me that he felt as if he needed to twist his testis to get more comfortable. Scrotal skin is quite stretchy and a what appears to be a half inch incision  may have allowed your testes to have been delivered into the operative field under anesthesia. Ultimately, if you continue to have problems, discuss a scrotal exploration or a second opinion.
Good luck!

Hope that you feel betterr quite soon.
