Testicles tingle and left testicle pain

For about two months my testicles/scrotum have been tingling. It is not constant but it occurs frequently throughout the day. It could also be described as a a slight burning feeling. More recently, my left testicle has a dull ache that radiates up into the groin. It usually occurs when walking at work or on the elliptical exercise machine. When touching the testicle it is very sensitive. I sit down and take the pressure off the scrotum and the pain goes away.  I saw my PCP who did not feel any lumps and the "cough" test was OK. He told me to wear "tighty whitys" when exercising but that seemed to make my left testicle hurt more often. Now,  when I urinate,I can feel the urine flowing through the scrotum and it tickles deep inside, followed by a lot of tingling.

Hi.

Was a urinalysis and urine culture done?

Was a prostate examination done to help rule out any underlying infections?

This may be a prostate infection, a urinary tract infection involving the bladder and urethra. Another differential for this is a musculoskeletal pain. A neuralgia that may be caused by certain exercises such cycling exercises which may lead to pudendal neuralgia may also be considered.

The sense of urinary urgency however, may suggest that an infection may be more likely than a muscle pain or a nerve related pain.

Now it's several days later and I always feels like I have to urinate, even when I've just went.
