Urinary tract infections, bladder outlet obstruction & urogical problems

17th January, 2010

Case History : Urological problems since 1996.
Patient : Anwar A. Khan, Sex : Male
Age : 56 years

A. Present Problems :

1. Frequent urine discharging, urine colour yellowish, burning in releasing urine, obstruction in releasing of urine, cant keep urine pressur(need to go to the toilet immediately to release urine) itching on the back and little pain on both thighs (muscles). Also an insomnia patient for long 30 years.

2. An USG of KUB, Prostate & PVR done on 21.11.2009 reveals that both the kidneys are normal, urinary bladder is regular in outline, wall thickness is normal, no intravesical lesion is seen. Prostate is enlarged in size (44X46X16mm). Volume is about 49cc. PVR is almost nil.

3. Microbiological Test Report (Urine Culture & Sensitivity) done on 09.01.10 reveals : Staphylococcus Haemolyticus -1.0X10/ml. Doctor’s comment : Insignificant growth.

4. Diabetic patient for about 2 years. Biochemical investigations dated 09.01.10 speaks of : Plasma Glucose (Fasting) : 6.6mmol/L, Glucose (2 hrs. after Breakfast) : 12.4mmol/L

5. Bronchial asthma patient since childhood which is also under control.

6. As per the doctor’s advise, if I take medicines like ‘Uromax’ or ‘Maxrin’ (Tamsulosin Hydrochloride 0.4mg) I feel good but after taking 5-6 capsules, my sex goes tremendously down. So, I have stopped taking them.

B. Past History :

1) April/May, 1996 : Suddenly I was suffering from burning and obstruction in releasing urine. I was then 42 years old. Optical urethrotomy was done on 31.05.1996 on a wrong diagnosis considering stricture urethra. Thereafter, I did not feel comfort. Rather, so many problems, such as, burning of urine, frequent discharge of urine, continuous pinching inside the whole abdomen, obstruction in releasing of urine with incomplete voiding, sometimes 40-60CC urine remained inside after discharging, severe thigh (muscles) & back pain, little pain on the whole of lower abdomen including bladder neck problem developed with me. Since then, I have been suffering urological problems.

2) June, 2001 : Lithotripsy was successfully was done to disintegrate a stone of 7mm in my left ureter in CMC Hospital, Vellore, India and the doctor advised me for EIU/BNI because of the bladder neck problem which has not yet been done.

Please advise me, if possible : What harm(s) may occur to me? How to get rid of the above problems? …for which I shall remain ever grateful to you.

With regards.

Having UTI for the past 3yrs with symptoms like: burning sensation, whitish discharge, back pain, unclear sight, skin rashes, weakness &others . Mcs reveals its staphylococcus aureus and at times spp. Taking so many antibiotics including: Augmentin, ciprofloxacin, ceftriazone, cefuroxime and many others. all to no avail. Currently undergoing a pelvis x-ray and sfa mcs. What effect could this have on my fertility and other vital organs like kidney which is part of d urinary system?
