What could cause pain in right testicle/groin area, normal ultrasound and no fever?

Hi. I am a 24yo male, ~55kg, 5 feet 8 approx. Taking no medication, drugs, alcohol, or tobacco. I have not had any kind of sex ever, so no STDs. I am a student and have a quite passive lifestyle, meaning no sports, etc. mostly sitting, walking or lying on my bed.

History: I have had the same kind of pain multiple times over the years. Two times, when I was a teenager, I went to the doctor (dont remember which side the pain was). He physically inspected me and just asked me to wear supportive underwear. After that I never went to the doctor for this and the pain happens like once or twice every couple of years.

On Friday (13th) I woke up with severe pain in my right testicle area. I do not remember the exact area of the pain but any movement made me want to cry with pain. The pain was the level of (and felt like) getting kicked down there. There was no swelling or redness or fever etc. Given the history I didn't go to the doctor and just wore tighter briefs so that there is less movement of testicles and thus less pain. The pain went away probably the next day but came back 2-3 days later and just stayed there. This time pain was different, movement didn't hurt as much and lying down and wearing underwear briefs helped with the pain a great degree. It felt like the pain is in the tubes that go up or the back of the testicle AND the groin area.

So last Thursday (19th) I got a doctors appointment. He asked me questions like discharge from penis, painful urination, swelling, fever etc. All the answers were negative. He then did a physical inspection, including coughing for hernia. He kind of pinched the tubes going up from the testicles on both sides (or perhaps the epididymis, I didn't see). The right side had a lot of pain. He said it is probably epididymitis. He scheduled an ultrasound next day (on Friday 20th) which is here: https://i.imgur.com/kn7tivk.jpg The results came out today (on Monday 23rd) and were normal.

Over the weekend (Saturday 21th) I noticed a swelling (but no redness) on the right side of the scrotum but it went away within a few hours and never came back. The pain increased and I also started having burning sensation in the tip of the penis AFTER urinating, the burning sensation was very low in intensity though. No discomfort or wanting to pee feelings otherwise. Sometimes I felt a discharge from my penis but when I looked there was nothing. My temperature was still normal (97.8-98.5 F).

I went in again today (23rd Monday). The pain is 90% gone (perhaps its because I haven't removed my underwear at all the past few days), no swelling and no fever still, burning also gone too. He gave me doxycycline monohydrate for 10 days and a referral for the urologist. There are only two specialists in town so the earliest appointment I could get is more than 2 weeks from now.

My question:

What is your opinion on what I have? Should I be worried?

Is there anything that I should mention or discuss with my urologist when I have my appointment after 2 weeks?

I am super worried and dont want to remove my tight supporting underwear briefs at all. I have a feeling that the pain will come back if I do so. Should I try doing that or would it be a bad idea? I normally dont wear it at home or while sleeping.

Additionally after returning home from the doctor's visit today, I have started to feel pulses of pain in the tubes that go up, on the other/left testicle. I thought that it could be just my mind playing games so I tried to feel around my left testicle. I found that the back of my left testicle has a painless mass that feels like a thick protruding blood vessel or perhaps a rubberband. This is absent in the right testicle, is this normal?
