What would cause tingling in the scrotum?

Hello Doctor,

I am hoping you can help me out.  I am currently working with a urologist, but he is having difficulty diagnosing the issue I am having.  For the past 4 months I've had an interminttent tingling on the bottom of my scrotum on a daily basis.  The skin is also red.  Tests have come back negative for all STDs and UTI, so we tried an ultrasound of the scrotum which came back normal (other than an epididymal cyst).  I also have enflamed skin around the urethra (red and a little raised).  This is also accompanied by a slight pain/discomfort at the tip of the penis around the opening.  My doctor and I are stumped.  

My questions are - have you ever heard of these symptoms?  What could cause a tingling sensation in the scrotum?  What do you think this could be, and what would you do if you were me?

Never heard of this without an outbreak of herpetic lesions in the small area involved. Definitely not the whole scrotum and the outbreak would occur shortly after the onset of tingling...a few days at most.

Thanks Doc!  My HSV 2 test came back negative, but my HSV 1 test was positive.  

Is there anyway that HSV 1 in the genitals could cause intermittent tingling in the scrotum on a daily basis for 4 months straight?

I would look at local issues such as your clothes soap, material of which your underwear is made, soaps or any other chemicals that you might use on your genitals. If you have changed any of these just before this problem started, then switch back. If there were several changes, then do only one change at a time and see over the course of a week or two what changes in your skin.
If you are sexually active, investigate any changes that your partner has made. Also,  if you are using condoms, have you changed brands or types.
How about your activities? Did you start bicycling more? Running more?, etc.
If you are not getting anywhere, get an opinion from a dermatologist.
As far as tingling itself, aside from local irritation, I think of neurologic issues (herpes, nerve root compression).
