large prostate and swelling

i'm a 21 male,my prostate is huge docs words.40 (kg) or however u measure.hard to urinate and burning and feels like something gets stuck in urine tract and lots of pressure and going alot.testicals are red and swelled especially around the sides.have signs of infection.protein was 300 in urine.muscle spasms in rectum and behind penis.also rectum swells or pokes out around rectum and is very painful.but goes back after few mins after bowel movement also lots of pressure.had fissure/inner hem. surgery 1 month ago both just made things worse now have trouble breathing at night.also lower abdemon pain and lower back pain.and ear and neck has been swollen for 2 years.what does it sound like?thanks

now the pain when i urinate is gone feel the need to urinate about 5 mins after but can't tell when to go,but i did have a small amout of blood in urine for a few days after the attempt of catheter(for cystogram)and now have blood in stool bright red first and dark red and bad lower stomach pains.and sharp pains in rectum maybe prostate like nerve pains.and rectal pressure and swelling together.doctor wants to do a scope,but im afraid from the other day's results and bad pain.what should i do?

need to add some things,i went today for a cystogram,and for over an hour they tried to pass the catheter to put dye in my bladder for the scan(screaming pain).they even tried a small tube but would not pass prostate it's almost swelled shut but i can urinate it is so painful,with burning and feels like razors mostly middle to end of penis.also testicals are red swelled and started sucking up and one was higher.so,they put the dye in my veins and waited for my kidneys to filter.still have pressure and swelling and all other symptoms.psa levels are ok i think and no mass from prev. ct test.im in so much pain what do i need to do?does this sound life threatning?please help,thank you

thank you,i had inner hem. and fissure surgery.also have black spots in stool mostly small round like big pepper(in urine to rarely) but a few bigger pieces sometimes,maybe bacteria got in fissure?also had catheter after surg.really can't find were to go for help.feel weak and shakey and lots of pain alittle worse every day and im on cipro and flomax and uta and ibuprofen.no 24 hr. urine did have ct scan of pelvic area and psa level test but hasn't come back.thank u for your help.

You had prostate surgery ? Were they able to send this for histopathologic examination?

This sounds like an infection however, we have to determine the focus of infection here. You have proteinuria. Was a 24 hour urine sample done to confirm this? Also, was a kidney,ureter and bladder scan done to determine any structural problems which may help explain the back  and abdominal pains? Presence of red cell casts, red blood cells and bacteria may suggest a kidney disease and an infection. Also blood tests to determine disemminated infection has to be considered. I suggest that you have this assessed by your doctor. A chest xray would also check for pneumonia or any congestion as you have noted difficulty breathing. The kidneys may be involved here. However other conditions like respiratory conditions  should also be assessed.
