severe testical pain

My pain is usually on the bottom fourth of each testicle and hurts more on my right than left.  The pain tends not to be severe unless contact is made or lots of movement.  There is no noticeable swelling. The only noticable symptom I have in that area is pain.  I also have had negative pressure in my ears for the same amount of time which may or may not be related.  

I went through the blood, urine test, ultrasound, and got swabbed for stds, twice nothing showed up. I have been on antibiotics for the last couple months the pain gets better, but never fully goes away.  Then when I stop the antibiotic it usually gets worse and repeats.

The Urologist that I have been going to says that if it was Epididymitis that it would have been cleared up by the antibiotics and is not concerned by the pain.  My primary doctor says that there aren't enough symptoms that match anything.  Could it still be  Epididymitis even though there is no swelling, redness, and the pain seems to be in the wrong part of the testicle?

The pain is on the rise again and I am up to 19 ibprofen a day to cope.  The pain right now has spread to the whole of the testicle. Does anyone have any ideas as to what this may be? Why do you think this?  What are some options?

Isn't it entirely possible that I never had Epididymitis; and that the cycle of on and off pain is simply what happens with whatever I have.  

My stress and emtional state is alot better than it was when I started I just quit a job that made me unhappy and started one that does.  I would be suprised if this is psychological, it would be easier to say it was if I was having flu syptoms for instance.

Is there anything else this could be?

Thats more or less what the doctors told me about my pain down there!!!Six different drs. later i have a condition called "pelvic floor disorder"! It wasn't all in my head!!My last 2 drs couldnt belive my other drs. couldnt diagnose this!

You have obviously been trying to take care of yourself. You have had all the tests and taken all the medications that your physicians have recommended, but you are not getting better. When I see this in my practice I have to look for  an emotional etiology, usually stress. The fact that things get better on antibiotics, but never resolve completely reinforces my feeling that you need to be evaluated by a psychiatrist/psychologist/social worker. Some family practitioners are also willing to entertain the emotional side of individuals in making their assessments. If you do not get to the cause, then you are only treating the symptoms and things will get worse and harder to treat.
Please be careful with ibuprofen. It is not a benign drug. It can kill kidneys and cause stomach bleeding amongst other things
