testicle problem. could ir be epididimytis or something else?

About 3 weeks ago I felt pain in my testicles. I wasnt able to sit, but overall I would rate the pain a 4/10.
The urologist thought it was epididimytis and prescribed me some anti biotics. After 3 days the pain and swelling were going down. In 7 days the pain was dompletely gone.

But a new thing started to happen. My testicle started trying to twist in either the verticle, horizontal or both directions. I noticed that when it did it there were these lumps that would appear. Always at the back of the testicle. They would appear either on the two poles, on the back, or on the cord. Massaging them they felt like very tense areas or semi hard lumps, but after massaging them for about 8-10 seconds, they would quickly shrink or reduce in tension. The twisting would stop. Sometimes the sensation feels quite literally as if my testicle is trying to rise because of temperature changes, but there's either a lump or somekind of tension preventing a part of it fromrising, forcing it to twist. Or only forcing that side to rise.

Again, when it happens massaging has been the most effective treatment. Lying down and resting helps alot aswell. Cold causes the tension to return. Very big heat causes lumps to appear and then a whack a mole game begins. A temperature thats only very slightly above body temperature seems to be perfect. Clears up the tension, clears up the lumps. Mixed with lying down and massages it feels like a miracle. The testicle up hanging like it always did after lying down and slowly warming up.

The problem is whenever I move too much or God forbid, SIT, it all comes back. Sitting is probably the thing that triggers the whole thing the most. Had it happen just now, the testicle tilted on the side and its lower side part had this rough hard flat patch. Like a tensed up and contracted muscle. Massaging it slowly relieved the tension and the testicle went back to normal.

Feels zero pain for the few weeks. Only tension and when the lumps get too big, slight pins and needless for a few seconds after massaging. No problems ejaculating, but it does slightly irritate the testicle. No problems urinating or anything else. The testicle reacts nicely to stimularion (going up and down) except the areas that tense up. Which again, massaging relieves. But it's annoying because I cannot sit and im afraid to walk far outside. Im afraid that the testicle will twist and end up in a torsion.

What could this be? My urologist is on vacation and I dontreally want to visit any other ones. Could it be because of epididimytis? To be completely honest I still havent given myself full rest over the weeks. Every 2-3 days I end up sitting down and the problem returns. Am I just retriggering the injury? I sit alot because of mh job and my doctor thought it could be caused because of it. Especially because the pain first started after I sat for so long and uncomfortably a few days before, that I felt huge pain after getting up for a few min. And I tend to sit alot during the days for many many years now.
