How are you? In youth, the infection can be due to sexually transmitted infection from chlamydia or gonorrhea and is associated with urethritis (inflammation of the urethra). In older men over 40, it is associated with urinary tract infections and prostatitis. Abstinence until the infection is cleared, is advised as with sexual practice there is a chance that you transfer infection to your partner.
Therapy is by antibiotics (often up to 3 weeks) and pain relief.

If it is epididymitis what can be done and what are the causes of this also are there any further concerns for myself and or my wife, should we avoid sex is it contagious etc.

How are you? There are quiet a number of causes which could lead to pain and swelling in the testicle, but confirmation can only be done with ultrasound and physical examination. The common causes are orchitis, epididymitis, varicocele and testicular tumors.
Also an enlarged hydrocele or spermatocele can cause dull aching and discomfort due to increased size. If the swelling and pain have developed over a couple of days then infection could be the cause.
I would suggest that you visit a urologist and seek complete work up and further assistance in treatment.
