Everytime we have sex, i rip? Help?

It was nearly 3 months of being sexually active with this guy (he is my first sex partner. So, 3 months of good healthy, non painful sex. But one night, having sex I got this shorting pain, so we stopped. Unable to have sex for weeks, I went to a doctor.. had all my STD & STI testing done and everything came back negative.. the problem went away for awhile then it was back again... my new doctor told me to keep it clean.., which I was kind of unable to do as I had an abortion and was bleeding for a month.. and then it was gone for a weekish. I get a small circular tear , kind of crater like at the bottom of my vagina (posterior labial commisure) now it's back. I'm getting really ******* tired of this... perfectly fine for 3 months and now sex is painful as ****. Doctor has thought it was yeast infection, urinary tract infection.. but it's not either. I have spent way to much money on prescription medicine. Please help me.. tell me what you think it might be.

I'm always wet when we have sex. And I've noticed pain and the tearing only happens when my legs aren't spread way open.. it's almost like it's just rubbing to much.. but it doesn't make sense, it never used to bother me and now it does.. HELP? my doctors seem to be useless

The sore is like circular.. not a slice

What do you think is happening?
My boyfriend also tells me to relax and I try, I think I am relaxed but maybe I'm wrong? Idk
