Unresolved Uterine Pain and Discomfort

Hi there, so I had the IUD put in in March, and started having funky symptoms in October (irregular discharge/odor). The obgyn diagnosed me with BV and chlamydia, which was a total shock because I've only had protected sex, but I tool the medications and the symptoms went away- tested negative after that. A little while ago I started having mild pain on what feels like NY right ovary- it comes and goes, and it's usually pretty mild. I've also had some overall pain, with it being bad in the morning once or twice but always disappating. That again comes and goes, often toward the top of the uterus. I've also been spotting, particularly after sex (I no longer get periods with Mirena), and will sometimes get an itchy vulva with strange discharge. I went to the obgyn again for the pain and she did both a physical and std/bacteria check, everything came back normal. Should I go back in? Does anyone have an idea what this is? Thanks for the help!
ps- I'm 21 and I have PCOS, although it was never confirmed through ultrasound.
