Very Heavy Cycles?

I'm 25 and for the past few years, I've had very heavy cycles with large blood clots. Sometimes I have to change my pads multiple times in an hour because the blood simply gushes. It's uncomfortable and makes it very difficult to leave the house since I do bleed through the pads and thus bleed through clothing very, very easily. The blood clots vary in size as well. Some are small, some are quite large. I'm not quite sure what I should do or what it could be?

See a gynecologist.  You should not be filling a pad that fast.  (Do note I'm assuming you're using the super heavy overnight ones since you have had heavy cycles for a few years now.)  As a general rule, if you completely soak a pad in over two hours, that's reason to see your doctor just to rule out any concerning causes.  If it ends up being nothing, you can discuss using birth control to lighten your flow by some.
