at a loss

Hi, ive always had a history of cysts and hormonal imbalance. I've never really had someone try to figure out what is going on. Since I had my momo twin girls in Nov 2012, I got a nexplanon implant. Since then, I've had irregular, heavy, month long periods with average cramps. This time, I'm passing numerous clot, cramping that takes me to my knees and today the pain is spreading to my side and lower back. I can't be seen till next week. Can you suggest anything? I've tried nsaids and heating pads. Thank you
