In trouble -need your expert advice NGU

I am 46 yrs had unprotected vaginal sex once with a friend 9 mo back, found she is promiscuous. Since I did not doubt of any STDs at that time,having normal sex life with wife. I was not that carefully observed but found no obvious symptoms in 1st month.
A month later tested in a lab, urine (no swabs) and blood for Gonorrhea,chlamydia,RPR,HIV,HepB,C, herpers1,2 and all are neg.
At around 60 days used Azithro for unrelated ear infection.
At around 3 mo, found slight pain on left testicle and GP gave Levoflaxacin 750mg for 10 days, pain gone.Urine for GC is neg.
At 6 mo, burning sensation on the penis head  and 1/2 inch from the tip, GP thought it is UTI and gave cipro for wk. Finishing cipro, tested urine for GC, herpes1,2 and RPR all neg.
Burning cont for 3 wks after cipro, saw urologist, who thought it was U.U (no tests done) and gave 2 weeks of BactrimDS. Problem cont. and wk after BactrimDS finished, upon my req urologist took a swab and tested for herpes2 and urine for GC, Mplasma and Uplasma from urine that was emptied 1hr before and results are neg. and no medicine advised.
Burning continued for around 2 months and at 8.5mon started feeling pain in urethra tip and mild occasional itch and urologist gave 2 wks of Doxycyclin100mg x2 for 3 weeks and Repaflo once a day.
1 Wk into doxy, saw a discharge before urinals which looks clear sticky little more thick than pre-***, panicked. Observing lately penis tip is wet sticky like thing, may be clear. Now my questions:
1. How accurate are my urine tests taken from urine that was emptied hour esp  for Micoplasma and U.plasma and GC
2. Am I co-infecting each other as my wife is not treated until now ( uro suggested doxy for both)
3. What do you suggest at this esp in the wake of discharge (may be clear )before urine which panicked me.
4.Why I am having burning so long? any other possibilities like HPV in urethra, etc
It is disastrous act for which I am suffering, need your expert opinion to bring me out.

The 2000 character limit is there for some good reasons.  Among them is that we really don't want nearly as much detail as you provide here.  An online forum is not a diagnostic service, and we don't need great detail to provide the level of advice that is appropriate.  On superficial scan, I see nothing in this long addendum that changes my opinion or advice above.  The symptoms you describe reinforce my belief against an STD.

However, the clarification that the discharge was in your partner, not your own urethral discharge, is important.  It is impossible for men, especially without medical training, to judge whether vaginal fluids are normal in either character or amount, so I discount that information entirely.  It has nothing to do with your symptoms.

The additional answers to the additional quesitons are:

1) Yes, that remains my conviction:  no STD.

2) No STD, therefore not applicable.

3,4) I recommend against any more testing of any kind, unless your urologist advises otherwise.

5) Based on what I know, I would not recommend any treatment for your wife.  But if my other judgments are wrong and in fact you had any of the likely STDs and infected your wife, doxycycline should take care of it.

6) I gave you my reasoning about no STD and have nothing to add.

That's all I'll have to say.  Please accept the scientific evidence and reassurance I have tried to convey.  If uncertainties persist, revisit your urologist.

Welcome to the forum.  I have good news for you:  I do not believe you ever had sexually acauired NGU or any other STD.  In the slight chance you did at one time, you surely no longer are infected with anything from your sexual encounter 9 months ago.

First, if you acquired NGU (or any other STD) from that event, you would have had symptoms within 2 weeks.  Second, you are at that age in men when prostate gland problems become increasingly common -- and most if not all your symptoms are consistent with prostatitis, which is not an STD.  Third, there are no STDs you could have acquired that would explain such symptoms and that could persist in the face of the antibiotics you have taken.  Your continuing intermittent discharge probably is due to prostatitis, which most commonly involves non-infectious inflammation -- i.e. does not respond to any antibiotic.  To your specific questions:

1) Ureaplasma and most mycoplasmas are normal bacteria in the urinary tract. Some strains may cause symptoms like NGU, but this is controversial.  In any case, all of them are covered by the various antibiotics you have had.  There is a theoretical possibility you have M. genitalium resistant to most antibiotics.  If you want to try another round of treatment, talk to your doctor about moxifloxacin (Avelox), which is uniformly active against MG; but I doubt it will make any difference in your symptoms.

2) I doubt you and your wife are sharing any infection that explains your symptoms.

3) Discuss your discharge with your urologist.  As noted above, I suspect it is prostate fluid, not necessarily indicating any infection at all.

4) Prostatitis can cause such burning.  As implied above, not all urinary inflammatory symptoms are due to infection.

So again, the good news is that no STD is the problem here.  I am quite certain that your problem has nothing at all to do with your nonmarital sexual encounter, and that your wife is not infected and not at risk for any STD. The bad news, I suppose, is that men with these problems sometimes just have to live with them.  However, sometimes prolonged antibiotic therapy has some benefit.  This is something for a urologist to address in person, not something you can expect to sort out by online.  Perhaps your next step should be to print out this thread and use it as a framework for discussion with your urologist.  I'll bet s/he agrees with all or most of what I have said.

In the meantime, don't be overly worried about this.  Such problems rarely if ever lead to any serious health problem for either affected men or their sex partners.

I just want to clarify any confusion in my statement would arise by saying "Saw milky white discharge (5-10ml) in her before. " in the first line. What I mean by that is - I saw a small amount of milk like white discharge in her vagina at that time I had unprotected sex with her. providing some hints if that may be any STD.

Thank you so much, not able to find words to express my gratitude towards your kind hearted noble service for the people suffering, struggling, and burning with this stigma. I am following your posts and needless to say how much one values you both for the needed help you both are providing.

Due to 2000 limit, I couldn’t present these additional symptoms and I highly value your precious expert opinion after these.

1.After unprotected vaginal sex, I washed with soap, will that have any effect of slowing down things? Saw milky white discharge (5-10ml) in her before.

2. Earliest symptom I recollect apart from the listed ones in my prev post is :  6-7wks, felt like minor burn while passing urine.  Thought a minor stuff and drank cranberry juice.

2.After 4mo, started appearing 6 to 8 black/brown flat patches on glens  (like pigments) varying 2mm to 5mm for every 4 weeks. Uro thought it is freckles and Derm thought lentigo (Dyschromia, Unspecifed).

3. At 6mo, 10 days prior to my penis head burning issue, my wife complained itching and burning and tried 3 day monistat (just in case) and her problems gone after a week. She did not think it was fungal but later she does not have burning. Stopped having sex for about 7-8wks.

4.From 6mo until now (10mo), started with burning of penis head, mostly afternoons and evenings, and for last 4weeks developed moderate frustrated pain in urethra and head(some times sharp sting like pain) and some intermittent itching around the genitals. At time when burning goes off, it feels like cold on the head. Now I have discarge.

5. If I pullback the foreskin and leave it back I feel less burning or minimal(may be due to skin holding the shaft tight). Uro says no fungal on the head.

6. The reason why I am worried co-infecting is, she does have burning issue at around 6months of the incident for about 10 days. Not sure monistat is the one removed her burning. I am taking antibiotic once every 6 weeks and having contact with her. Every time I have sex, the next day morning I have minor itch around the shaft, etc.
The urologist recommended to take doxy both of us, do you think it is good enough even  in case any we share some infection.


No my worries:
1. Do still believe no STDs given additional info
2. If it is STD, do think the burning and pain in urethra/head is intermittent like this  only in evenings mostly.
3. In the wake of this discharge, do suggest any more tests to be safe?
4. What is the best way of testing Micoplasma Genitalia, do you recommend testing.
5. Even if any remote risk wife has, would Doxy for 2 wks takes care every thing?
6. Any other logical explanation you provide with reasoning behind no STD thought brings me peace in my life.
