Prominent Common and Intrahepatic Ducts,Gallbladder Distended but No Stones

I am so grateful to have found your forum. Can you please give me your professional opinion on what symptoms and Ultrasound results could indicate?  My symptoms are so varied, my doctor has no idea what's wrong with me.  I'll likely put a lot in point form since there is so much to cover.

I am a 42 year old, Caucasian female.  Low to average BMI. Medium build, muscular arms. (I used to work out a lot)  Now, I look very sickly - skeletal from the rib cage up but have a very distended stomach and look 4 months pregnant. Normally, I have a pretty flat stomach. I weigh the same as usual though. 135 at 5' 7.  I have had high blood pressure since I was pregnant, 12 years ago and take medication to control it.  And I have severe bowel issues.  My normal pattern of bowel movement is every 20 - 30 days.  I've tried everything.  Increasing my fiber only causes a lot of pain.  I think I've lost the natural contraction 'feeling'.  It is not pleasant and even laxatives don't really work.  I only use them around the 25 day point when I'm really suffering.

My recent Ultrasound Results - Gallbladder is distended and Common Bile Duct is prominent at just under 9 mm. Some prominence of the Intrahepatic Biliary Tree as well.  This should be correlated with the patient's liver enzyme profile. No gallstones were identified within the biliary tree. Spleen, kidneys, liver and visible portion of the pancreas and abdominal aorta were unremarkable.

What does that all mean?

All my symptoms began within the past 6 or 7 months.  My hair started coming out in clumps, floating in the water of my bath.  I have a bald spot now and it's still falling out.  Night sweats, hot flashes, low grade fevers that would come and go throughout the day, jaundice most noticeable around my mouth (but I can't see it now)  Terry's Nails.  My doctor calls my pain, Neuropathic pain and it's everywhere in my body - skin contact, joints, muscles (although I do have Fibromyalgia and Myofascia as well as Scoliosis).  Having my blood pressure taken really, really hurt and caused blue spider veins to pop up in my hands.

Sudden, acute Phlebitis in arms and legs since April 15th. Extremely painful from hips to feet.  Even cloth lightly brushing against my legs causes a yelp of pain and I've almost passed out. New spider, varicose and large blue veins are popping up all over my legs, feet, forearms and hands.

Over active Thyroid diagnosed in March 2013 but I had symptoms of both Hyperthyroidism and Hypothyroidism.  Naturally cured itself by April 2013.

My Cholesterol is moderate to high for the first time in my life. I never eat fat or greasy food.  Have always eaten Clean. No sweets, hardly any meat at all.  Love vegetables.  (eating disorder since I was 15 years old.  Anorexia Nervosa with Bulimic tendencies)

Tested as having either No Immunity to Hep B or acute infection at the time tested but my doctor wasn't concerned  < 3.1  Range was   9.9

Bilirubin:   9 in Mar. 12 in April. Range is:   < 22
Fasting Blood sugar usually really good at  3.5 and is now 5
My Potassium Low. My ALP is Low (Alkaline Phosphatase)
Creatinine - 74 Mar  88 in April     Range: 45-97
eGFR - 74 in March   61 in April  (with flank pain)

I've declined a lot since I last had blood work done.  I'd really like to have it done again. My doctor said it might be worth checking liver enzymes again but never gave me a requisition.

I'd really like to know what you think of my case, in particular my ultrasound results because I haven't eaten anything for a couple days and I don't know whether to go to the emergency room or not.  I have made a couple cups of Flax seed tea and researched what I could eat if experiencing a Gallbladder attack but I don't know how to treat myself because I don't know what is wrong with me.  Your opinions would really help direct me.    Thank so much.

Hi there!

The report describes enlargement of the gall bladder and the draining canaliculi, which could occur secondary to growths/ masses or due to a primary defect in the biliary tree as well and may require further investigations to arrive at a definite diagnosis. The symptoms described may or may not be related to the ultrasound finding and other possibilities such as infections, inflammations, hormonal/ endocrine imbalance, other systemic issues etc. would need to be ruled out as well. I would suggest considering a detailed evaluation by a gastroenterologist and an internist and depending on the cause diagnosed/ suspected, a management plan can be chalked out accordingly.
Hope this is helpful.

Take care!
