Irritation surrounding meatus opening

Irritation/redness around top of meatus opening...tested for chlamydia/gonn/herpes, all negative. Two separate urine tests came back clean. Was given a wks worth of cipro at first (small amount of bacteria, Nurse Pract said possible UTI (after uti urine test).....then was given 5 days worth of flagyl a month ago (for possible trich), and over a week ago was given macrobid for only 3 days (felt better for a couple days). Have seen 2 urologists, both said I'm fine and nothing is wrong. At times it feels like a burning sensation, getting worse when I have to urinate. No discharge or burning while urinating. Any type of antibiotic to try or cream? It's been over 2 months and it's driving me crazy.

Feel it may be yeast, since the redness/irritation seems to only be at the very tip of the penis...I've been trying lotrimin the past couple days just to see the results....thoughts?

If it's just an irritation (which seems strange since I started noticing it after unprotected sex/oral from a girl.....she says she feels 100% fine)....what type of cream would be good to use on the tip? Would I need some sort of steroid cream, or would something OTC work?

Thanks for the response.

As I mentioned, I've been to two local urologists, niether have been very helpful and both said "you're fine, stop thinking about it". The second perscribed me some macrobid, just to leave me with a peace of mind. I actually did feel better the following three days, but the irritation returned shortly. It looks like an infection/irritation to the urethra (though there is no painful urination, discharge).....but given where the redness is (right around the tip), I'm not sure what else it can be.

Since Monday, I've been applying lotrimin to the area in case it's a fungal infection/yeast. I go to see my GP (again) next week, but once he tried in the beginning, he refered me to a urologist (which were no help). Is there anything he can perscribe me?

Thanks for posting your query.

I can understand your concern for such redness, irritation at the tip of the penis. Most of the STD’s and infections causing these symptoms have been ruled out. Some possibilities which I can think of and which need to be ruled out are:

1) It can be due to local irritation of the area due to continuous rubbing of the tip of penis with the clothes. This can cause chafing or friction rub. It can be due to friction rub caused by sexual activity as well which is exaggerated by the touch of urine.

2) Secondly it can be due to balanoposthitis. Balanoposthitis is an inflammation of the glans penis and the foreskin. It presents with red, erythematous inflammation of the glans and tip of penis. It is usually caused by bacterial infection and should be differentiated from fungal infection or candidiasis.

3) Third possibility although less likely at your age is of prostatitis. The typical symptoms of prostatitis are fever, pelvic, testicle, or rectal pain,  and painful urination and ejaculation and incomplete emptying of bladder But sometimes it may present with redness, irritation and burning of the tip of the penis.

You need to get evaluated for these three possibilities from your urologist. Some investigations may be needed by your urologist and these include KOH examination of the skin scrapings of the tip of penis and fungal culture of the skin scrapings to rule out any fungal infection, bacterial culture of the skin scrapings, digital examination of the prostate, prostatic specific antigen and an ultrasound of the prostate.

Till the time you get t examined, wear cotton clothes, wash the area with fresh water and avoid any intercourse or masturbation. Also during intercourse in future you may use water based lubricants like KY jelly.
