pleade advice me if I need drug

Hello Sir,
I was diagnosed with Hepatitis B in May last year and immediately was placed on Lamudine tenofovir disopril fumarate a combination drug(3 in 1) which I used for almost 8months.
I did the test after 6 months and it's still there but I wasn't feeling any discomfort.
I later went to see a consultant on Hepatitis B who asked me to do several test which the results are attached here.After the test he the consultant told me that I don't need drug a that all my tests are normal.
But stopping the drugs I noticed symptoms like feeling weak,pains in the Abdomen,back,neck,shoulder which comes and go away I complained to the Specialist which he said they are not part of the symptoms of hepatitis B.
Please what can I do.

My test results with first doctor after testing positive to hepatitis B in May are as follows: ALT 18.7,AST 21.4,ALK 26.3,Total bilirubin 13.6,Conj Bil 2.04,total protein 68.3,Albumin 54.2 while after using Lamivudine Tenofovir Disopril Fumarate (all in 1drug) for almost 8months is HbsAg positive,ALT 12,ASR 19,Albumin 42,Direct Bilirubin 14.5,total bilirubin 27.5,total proteins 90,FBC,platelets counts and ESR shows normal and Ultrasound scan of Liver in CHB is normal also according to the new hepatitis B consultant.He said I don't need any drug and that I should discontinue the lamivudine Tenofovir Disopril Fumarate drug.please what do advice I do becos I began feeling a bit weak sedomly,and muscles and pains in my back,shoulder,and Abdomen once in a while buy changed position.Please I want to also know if am acute or chronic carrier and my chances in clearing the virus and if I used drugs. Currently I use only Silibon70mg.

Hi cosbyosomo it looks like you are chronically infected with HBV. But the really good news is that your ALT and your HBVDNA are low, so right now you don't need medication. Unless your medication helped you get to this level? People need medication then their ALT is elevated for example consistently above 35 for men, 25 for women, and also your HBVDNA is above 2,000 IU/ML. You are way below this.

Regarding the pain you are describing, a lot of people complain about this, weather its related to to your liver or not we do not know. You could be getting a side stitch, it could be your gall bladder. The important thing is that you get your ultrasound done, and  your fibroscan done.

1. Fibroscan get this ever 2 or 3 years
2. Ultrasound, get this annually if you are under 40-male/50-female and every 6 months if you are over 40-male/50-female.

I got today the actual result of my AFP=2.0ng/ml(0-8.5) and Hep b panel(HbsAg=+ve, HbsAb=-ve, HbeAg=-ve, HbeAb=-ve, HbcAb=-ve. I will post my new LFT tomorrow result pending and ultra scan.

My LFT result is AST= 20IU/L(0-50), ALT = 21IU/L (0-50), ALK= 54IU/L (40-130), Total Bilirubin = 13.8umol/L (0-19), Conjugated Bilirubin = 2.4umol/L (0-4.3), Total Proteins =70g/L (60-80), Albumin = 39g/L (35-50).

Also My Ultrascan is normal (The Live measures 13.5 i its span. Its has smooth regular outlines. The Parenchymal echo texture is preserved. The intra hepatic extrahepatic ducts and vascular channels are with normal limits. The Gallbladder, Pancreas,Spleen and both Kidneys are sonographically normal but there is probe tenderness over the epigastric. The bowel loops are normal in calibre demostrating normal peristalsis. Normal Scan, ? Peptic Ulcer Disease

I received the result of other test today.

LFT: Serum total bilirubin 25umol/L(2.0-21),AlT:39iu/L(<40),Albumin:41g/L(35.0-50),ALP:56iu/L(30.0-130),AST:28iu/L(<40)
Serum gamma-glutamyl transferase:32iu/L(<90)
One stage prothrombin time:12s(9.0-14)
Serum AFP:<3ku/L(<7.0)
Hepatitis B DNA PCR: 1.21*10^3IU/ml.

I am not a doctor. You have given us very little information. You should have test results for your viral load (hbvdna). It is definitely not necessary to take Lamivudine at the same time as Tenofovir. Tenofovir on its own is enough. The symptoms you describe are not specifically related to Hepatitis B. Silibon(Silybon) is just milk thistle, a herbal supplement, it will not cure your HBV.
Provide more test results, such as HBsAg, HBeAg, HBeAb, HBcAb status, as well as hbvdna results, will allow us to give you a more informed opinion.

Thanks I did HbsAg again on the 29th of Dec and is positive and will soon have my HBV dna but d last LFT in Dec is ALT 12,AST 19
HBV DNA 61.5,HbeAg -ve 0.06,latest LFT resuts are Total bilirubin 31umol/L(3.4-17.1),Conj/Direct bilirubin 28umol/L(0-6.8),AST 36IU/L(0-37),ALT 24IU/L(0-40), Total proteins 79g/L(<80g/L),Albumin 35g/L(35-50),ALP 24IU/L(21-105).Ultra scan of the liver normal.
Please me about my status and thevreason for high bilirubin.Thanks

HbsAg=+be, HBeAg=-ve, HbcAb=-ve, HbcAg=-ve, HbsAb=-ve, AFP(alpha feto protein)=-ve
From my understanding, you are still infected. Based on the timelines, it would appear to be considered chronic by definition and treatment should be considered depending on Viral Load levels / symptoms / LFT’ scores. There is still a chance your body will subdue it itself but regular testing every 3-6 months should be considered
