ORAL and PEP

Hi All,

I received unprotected oral sex (felatio) & protected vaginal sex from a female 53 days ago
Approximately 23h after event i requested PEP (Truvada generic + RAL). All the doses were taken for 28 days on time (truvada 1x dialy, RAL 2x dialy).
There were quite severe side effects over course of PEP treatment. It started with heavy headaches, fattigue, loss of appetite, depressions, mood changes, suicidal thoughts,to metallic tongue and rash in form of blisters. The blisters would show up only at the end of the cours on 4th week. They were always individual ones (no patches). In total 8 of them, they would appear amd dissaper after few days.
Right after the course i got tested by 4th gen test = negative.

Once i stopped the treatment the side effects remained for another 2 weeks. I found a research saying that elimination half time is 6,2 days after 28 days of PEP course. On week 2 post PEP after eating a sour pear a white dot looking like 1 single swollen taste bud appeared, which would dissapear after 3 days. After that i had another reaction to cabbage which made about 5 red dots on my tongue. Is it possible that this is allergic reaction to PEP concetrated in my tongue?

Now i went for 4th gen test 20 days post pep. Will this test have any value?

When shall i get tested next?

Truveda is Prep (prevention of HIV that you take before an exposure but must take it over time). If you took it because you had an oral exposure, shame on any doctor that prescribed that for you. That's uneducated. Risks for HIV are to have had unprotected vaginal or anal sex or to share needles for IV drug injections to get high. Air and saliva inactivate the virus. You needed NO treatment and NO testing as you had no risk from getting hiv from oral sex. Major over reaction and whomever prescribed that did NOT follow protocol of using the medication. Maybe because you were so anxious and they wanted to appease you but doctors need to be able to say no to anxious patients. You don't need to test at all. You didn't get HIV from oral sex.

Truvada + Ral was taken as Post Exposure Prophylaxis where you take 3 drug regimen instead of 2 drug regimen for Pre exposure Prophylaxis. The doctor mentioned that this is against protocol and not prescribed regularly after oral sex. It was my own decession to take it as the BJ lasted for about 1 hour :(
A doctor who did that was being irresponsible. There is a reason why they should not let those who haven't been to medical school be their own doctors. :>)  You had no risk. And definitely did not need that medicine.

Hi All,

It might soubd ridiculous, but anxiety is taking grip again. I got tested by ag / ab in weeks 3,4,5,6 intervenously always with negatibe results. My doctor prescribed me to show up for ag / ab in week 8, which is this week. Last weekend i was kissing with someone. 1. Could i potentionaly transmitt it to her?
My son 2years old just got today fever. There is nothing then standar contact such as touching him during moving him to stroller, chair etc. 2. Is there any chance i would transmitt it to him?

Hi All,

Thank for great work you do here!!

I understand your answer about no risk via oral. Nevertheless, anxiety is taking over me and only way how i can relief is to get tested.

Please, can you advise on timming of the test after PEP?
My doctor told me to get tested by 4th gen test 8 weeks after last dose of PEP. This feels like ages. British guidlines order to get tested 6 weeks after last PEP dose. Here in your forum its reccomended to get tested by 4th gen test 4 weeks after exposure (for non pep users).

Please, what would ne your opinion? Shall i take into consideration long = 6,2 days elimination half life of PEP. That would mean that still 2 weeks after finishing of the PEP course you would have 25% of original concentration.

That's what anxiety does and glad you recognize that. You need to stop looking stuff up, stop asking and stop the circular thinking. The more you do, the more the anxiety will stick around. You do not get HIV from oral. Talk to your doctor, get rid of pep. You never should have been on it. Original opinion stands.

Two more notes please. There was involved also deep kissing. While one of my wisdom teeth was extracted 6 years ago and it seems that the hole never got filled as with my other extracted wisdom tooth. Nevertheless, it is never bleeding.
Second issue i have is since the exposure day my saliva is a bit more sticky, but still white.  It sometimes irritates me in throat, but this is ongoing already for some time and in meantime i had 1st 4 gen test negative.

Do you think that these 2 issues could be a problem?

Kissing is never a risk. Even with cuts, bleeding, recent procedures. Air and saliva both at play that inactivate the virus. No risk and no reason to test.
Thank a lot you for clearing this out GuitarRox. Anxiety plays big time with me as i can see some pimples. Might be cause by antidepressives i am taking or the anxiety itself. Dont you guys here offer paid online consultations, lets say over Teams, etc.?
No, we don't offer anything like that and you don't need it. Talk to your own doctor about anxiety. Medication for anxiety can be modified or increased if it is not working properly. Pimples are part of life and everyone can get them. Seriously analyzing your body and looking for signs of HIV or other illnesses is part of the anxiety. Therapy is needed and modify your medication if necessary. HIV is not a concern here.
Your situation involves personal contact with an object in air  (hand,  fluids,  maybe blood, cuts,  mouth etc. ). You will be happy to learn that you had no risk, because you can't get hiv from personal contact except unprotected penetrating vaginal or anal with a penis, neither of which you did and you didn't share hollow needles to inject with which is the only other way to acquire hiv - there are ONLY 3 ways to get hiv. Note that 2 of them require a penis and the third requires a hollow injecting shared needle - there are no OTHER ways to get hiv. Analysis of large numbers of infected people over the 40 years of hiv history has proven that people don't get hiv in the way you are worried is a risk.
Hiv is a fragile virus in air or saliva and is effectively instantly dead in either air or saliva so the WORST that could happen is dead virus rubbed you, and obviously anything which is dead cannot live again so you are good. Blood and cuts would not be relevant in your situation since the hiv has become effectively dead, so you don't have to worry about them to be sure that you are safe.
There is no reason for a person to test when they are safe. The advice took into consideration that the other person might be positive, so move on and enjoy life instead of thinking about this non-event. hiv prevention is straightforward since there are only 3 ways you can become infected, so next time you wonder if you had a risk, ask yourself this QUESTION. "Did I do any of the 3?" Then after you ANSWER "No, I didn't" you will know that it's time to move on back to your happy life.
No one got hiv from what you did during 40 years of hiv history and no one will get it in the next 40 years of your life either.  You can do what you did any time and be safe from hiv.
The other person's status is irrelevant when you have no exposure to live virus. You can't get hiv when you use a condom, and if it failed it would rip wide open down the seam so that didn't happen.
No one ever thought hiv could be spread by kissing so you're 40 years behind the times, and that and all the other non-events you are worried about (dots on tongue which prove ZERO about hiv) show that you are a very anxious person. This makes me think the doc prescribed PEP to get you out of his office. It is time to move on, instead of being worried any more about your non-event.
