Prostrate Surgery/Bleeding from Bowel After

My father in law just came home yesterday from having Prostrate Surgery, somewhat confused as he has had a Cerebral Brain Hemmorage in 07, dysphasia, unbalanced. He is 78. Was awake throughout the night couldn't sleep, pain. Gave him his pain meds. By early morning had bowel movement which is out of the ordinary for him as he has had Bowel Cancer and normally has to take some form of laxative to help him. However when he had Bowel Movement for the most part it was alot of blood and clotting. He was quite weak and not to coherant. I have homecare and nursing so I called the nurse as she was coming out in the A.M. When she arrived she insisted that he go to emerg because of his B/P and confusion, she called ambulance. He ended up in the Emergency Trauma Unit, had to be givin 2 units of blood as his white blood cell count was down (sign of problems) then being sent to ICU. The Gastrointestinal doctor will see him in the morning and do testing. I called the Urologist who performed the surgery and he said it wouldn't have anything to do with the surgery that had been done. He is sedated in the Trauma Unit at present until a bed becomes available this evening, the nurse did comment that there is bleeding going on somewhere in the body. Has anyone had this experience with a family member/yourself of this happening? Any comments would be greatly appreciated. I am myself a Health Care Provider but have never came across anything like this happening to those that I care for. I do know that there is cause for concern but what would cause such a thing to happen.
I might add that while he was in the hospital with cathedar I did not notice any clots or bleeding in the cathedar bag, a little pinkish right after surgery, the next day looked just like water. Could the nurse's have irrigated to much or did Dr. puncture some part of the bladder,bowel etc. Again any response would be appreciated, we as family are very concerned. Thank You!!

I have since found out this a.m. that  the upper GI Physician is going to do a scope tomorrow. He did mention however that since he has already had bowel cancer that if there is alot of narrowing it may be possible that he could get a tear from the scoping. I do know from looking at an x-ray quite some time ago that they said he had alot of stitching from the surgery and he does have alot of narrowing from surgery. The size of his bowel movements that he has with help of Lactulose (or else he can't go) is very small to say the least. What would be the chances of a tear since he is so narrowed due to surgery. My father in law is still in hospital, had to have 2 blood transfusions in Trauma Unit. He was then sent to ICU and now in a regular room. This is now day 5, they still have him on anti biotics via intravenous however, they are not saying why. He is to be seen by an Upper GI Doctor which we have been waiting for for 3 days now. My understanding is that he is going to do a coloscopy to find out where the problem lies. I had taken his meds in to Emerg. they aren't even giving him the meds he's supposed to be on as in his 2 Trazadone, knew nothing of him taking Ativan in am./p.m. which I just found out today. They have no recognition of where his meds are that I took in either. I am getting to the point of screaming at them. My father in law is getting very irritable with the not knowing and not understanding since his front lobal cerebral anurysm. How can I get through to these ppl. I am going to put in a phone call to his floor Physician. My husband went to see the nurse tonight to find that they are not giving him his 2 Trazadone (as per Dr.orders) and a mild sedative. I don't understand why they won't give him what the pharmacy has said he is on. This is getting quite stressful for the family but most of all my father in law. He has already taken out the stints that they left in his neck for transfusions (very irritating). Can you tell me as a professional what I can do to make things run smoothly and what right questions to ask. I seem to be getting nowhere and am becoming irritated myself an trying to calm my father in law. Thank you in advance,

Hi,
     How are you? Blood in bowels after TURP, is not related to the surgery, but could be due to preexisting conditions like rupture of vessel or an neoplasm inside the bowel or due to derangements in the coagulation profile. The latter one could be the most possible reason as he turned incoherent after surgery as this could be due to stroke.
I would suggest that you get a CT brain, coagulation profile and colonoscopy done to assess the condition. The other reason for incoherence after TURP is imbalance in sodium concentration in the body.
Discuss with your doctor regarding all these possibilities and follow his advise.
