Hi, I'm 17, I'm going through some simalir things as you have. I know I'm not a doctor, but here's my advice for what your going through.
if you feel you forget alot of things try carrying a planner type thing with you, or documenting every possible thing into your phone, or try Memiary. Memiary is a web based personal Diary, you don't need to share anything it's not intended for that. It's easy to keep up because you can use it from your phone via text messages, and it's got one main thing that actually makes it feel do-able, it's simalir to twitter in the way that you have a word limit to your entries, but you get 5 entries a day, this makes it so your mind sees that you don't need to write alot every day, but you have the ability to if you feel you want to!
