11 years old!!  He has reached a phenomenal age!

Yes, you should take him to a vet who is familiar with rabbits.  The vet should perform an exam, blood work, and X-Rays, if necessary.  Unexplained weight loss could be caused by many different abnormalities.

Due to his advanced age he could be losing weight from causes such as kidney or liver disease, or degenerative joint disease,  and many more.  DJD, or Kidney, or Liver disease can be treated to slow the progression at the very least,  so do not be discouraged.

It could of course be unrelated to his age, such as gastrointestinal parasites, or a urinary tract infection which are easily treated with medication.
