my 10 yr. old took all stuffing out of bed

10 yr old wipes feces on himself and the wall he also took all the stuffing out of matress

My heart goes out to you as you have a huge challenge.  One of my dear friends has a son, now 19, who did the same thing.  She has tried all sorts of things along the way to help him (he still is in protective underclothing and dangles string, jumps around) but he doesn't smear anymore.  It was a phase he went through and she let him know that it was not appropriate.

He would tear things apart also with stuffing and chairs, blinds, etc.

It was an ongoing process to change the furniture to things that were not so easy to take apart plus having things that were patched up initially.
