Chronic Back Pain

I'm a 33y/o male who has two back surgeries. The first surgery (1999) was for a herniated disk (C6/C7) and the second (2002) was for spondylolisthesis (L5/S1)

Since the cerviacal fusion I have very little pain related to that procedure . Unfortunately though this hasn't been the case with my lumbar spine (TLIF,laminectomy). To help manage the chronic/ nerve pain my anestheologist has me on several different pain medications(dilauid,fentora,marcaine,opana) but sometimes when I experience "breakthrough pain" the relief can minimal. Surgery and medications have not been my only couse if treatment. I have gone to multiple doctors for their opions, test have been preformed i.e ct's,myelogram,emg (which indicated nerve damage of L5/S1 nerve root),physical therapy,selective epidurals,nerve stimulator trial,meditation,etc.

If anyone can relate to my situation or has any advice I would love to hear from.
