post epidural

I had an epidural to relieve pain from a fractured vertebrae in my lower spine L1 level and the 2 above it have degenerative disc disease. I have been diagnosed with severe spinal stenosis also at the L1 level.  I have constant pain and numbness in my back and down the legs.  Left leg to knee and right leg to the foot.  Since the epidural I have been disoriented and nauseated I have developed frequent headaches and vertigo.  I woke up during the procedure and got up on all fours due to the pain i felt during the injection. I was unable to get off the couch for 2 days afterwards and I could not stand up straight.  This all stemmed from an auto accident caused by a drunk driver 1 1/2 yrs ago.  I have not had more than 5 hrs sleep a night since this accident and typically only get 3-4 hrs a night.  I do not nap at all during the day.  I am mentally and physically exhausted, the doctor will not give me another epidural and I don`t want one at this point.  His recommendation is surgery which I am terrified of.  I have had no pain relief from the epidural injection and feel worse since having it.  Please help.

Are there any alternative treatments I could try first?
Are there any recommendations to help me sleep?

I forgot to mention, the epidural was done 3 weeks ago.
